Whenever damage to hull plating or structural members that may affect the seaworthiness of a vessel is suspected, the cognizant OCMI may conduct an internal structural examination in any affected space and may require the vessel to be drydocked or taken out of service to assess the extent of the damage, and to effect permanent repairs. Vessel allided with fuel pier piling on 01Jun2024 at Oak Bluffs. A damage survey will be conducted to determine if any damage to the vessel occurred.

The use of plastic tie wraps must be limited to bundling or retention of multiple cable supports, and not used as means of support. Replace all cable and wire throughout the vessel that is supported by plastic zip ties with metal supports by 13 MAR 2024.

Immediately after addressing resultant safety concerns, the owner, agent, master, or person in charge of a vessel involved in a marine casualty shall notify the nearest Sector Office, Marine Inspection Office, or Coast Guard Group Office whenever a vessel is involved in a marine casualty consisting of: (1) An unintended grounding. Vessel did not report grounding as required and continued on it's next scheduled trip with passengers prior to notification. Damage Survey required in the presence of the Coast Guard Marine Inspector, and a sea trial may be required pending results of survey.

If the alarm signal has not been acknowledged within 2 minutes, an audible fire alarm must be automatically sounded throughout the crew accommodations and service spaces, control stations, and manned machinery spaces. Manual call points did not activate to accommodation spaces.
