The company should ensure that all personnel involved in the company's safety management system have adequate understanding of relevant rules, codes and guidelines. The operational nature of deficiency 01 provides objective evidence of a failure of the ISM code. An internal audit report satisfactory to the OCMI must be submitted within 90 days. PSCO observed crew was unable to follow established procedures while operating the oil content meter on the oil water seperator and while calibrating the portable gas detectors. 74SOLASIX/3.1 ISM Code 6.4

Such monitoring equipment shall also provide an alarm facility which is activated by detection of over-pressure or under-pressure conditions within a tank. The machinery, boilers, and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequete for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to...other hazards. The vessel experienced a bow thruster casualty while transiting the navigable waters of the United States which adversely affected its maneuverability and posed a hazard to vessels and infrastructure along the houston ship channel. 74 solas(14) II-1/26
