(a)Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Operators found metal shavings in the reduction gear. Upon further investigation the bearing for the reduction gear failed. Operators took vessel out of service until bearing can be replaced.

Piping subject to internal head pressure from oil in the tank must be fitted with positive shutoff valves located at the tank. Power operated valves installed to comply with the requirements of this section must meet the following requirements: Valve actuators must be capable of closing the valves under all conditions, except during physical interruption of the power system (e.g., cable breakage or tube rupture). Fuel storage tank valve 4P failed to close from the remote actuator. Valve to remain closed when vessel is operating. Opening of valve permitted when transferring fuel to the settling tank and closed after transfer is complete. All manual opening and closing of the valve 4P to be logged in the vessel's official log book until valve is repaired and operation is to the satisfaction the attending MI. 46 CFR 56.50-60(d)(3)(i)

Each vessel must carry lifebuoys approved under approval series 160.150 as follows: At least two lifebuoys, each with attached self-activating smoke signals, must be stowed where they can be quickly released from the navigating bridge and should, when released, fall directly into the water without striking any part of the vessel. Bridge lifebuoys are of the manual cast loose type and have a water light attached. Replace lifebuoys with ones described above at the navigating bridge to the satisfaction of the attending MI. 46 CFR 199.70(a)(v)

The vessel must operate in accordance with applicable laws and regulations and in such a manner as to afford protection against hazard of life, property, and environment. Towing vessel tow line parted, then vessel collided with barge causing damage to barge port mid ship and towing vessel port bow. Damage assessment could not be conducted from interior of vessel due to CG officers not able to board vessel because of ocean surge/high tide and confined space could not be entered. Make confined space safe to enter. Provide video and written statement from Captain of vessel regarding damage to port bow from interior void of towing vessel with any tears, holes, damaged welds, and other damage noted and submit to Coast Guard prior to departure. 46 CFR 140.205(a)

Test procedure documents must be in a step-by-step or checkoff list format. East test instruction must specify equipment, specify equipment status, apparatus necessary to perform the tests, safety precautions, safety control and alarm set points, the procedures to be followed and the expected test results. Test procedures performed by service technicians differ from written periodic safety test procedures. Specifically, PSTPs call out removing the sensor and using oil bath or pressure device to test sensor and technicians utilize computer manipulations. Update PSTPs for manufacturers testing instructions and submit to MSC for approval, or provide testing capabilities as currently written in PTSPs.

Discovered steady stream of water from port main engine shaft seal. Demonstrate proper operation of shaft seal in accordance with manufactures instructions/guidance to the attending marine inspector. Provide specifications for allowance of water from installed shaft seal.
