PSCO observed lifeboat #2 and 4 unable to complete full range of motion on rudder during steering test.74 SOLAS 20 III/20.2/ Code 17AC/ Corrected on the spot

Electrical installations shall be such that: the safety of passengers, crew and ship from electrical hazards will be ensured. PSCO observed unauthorized electrical installation inside the AC room. 74 SOLAS II-1/40.1.3

Cylinders must be secured and when not in use they must be stowed in a rack in an upright position with the valve protection cap in place. PSCO found several compressed gas cylinders without valve caps, and multiple empty propane gas cylinders of different compressed gasses outside of the storage.

Where the contents and use of a space are such that there is a doubt as to its classification for the purpose of this regulation, it shall be treated as a space within the relevant category having the most stringent boundary requirements. PSCO observed combustible materials being stored in the shaft room which is a category 10 space that does not have fire suppression for that space due to its category. Also, the storage is not displayed, nor identified in the space categorization plan. 74 SOLAS 81 Amend II-2/26.2.2

A sufficient number of lifejackets shall be carried for persons on watch and for use at remotely located survival craft station. The lifejackets carried for persons on watch should be stowed on the bridge, in the engine control room and at any other manned watch station. PSCO observed that the lifejackets being stored on the bridge has CO2 cartridges that were expired. Lifejackets did not have vessel proper labeling. 74 SOLAS 97 III/7.2.1.2

Hinged fire doors in the stairway enclosures, main vertical zone bulkheads and galley boundaries which normally kept open shall be self-closing and be capable of release from a central control station and from a position at the door. PSCO observed that fire screen door 624 did not close. 74 SOLAS II/2 41.2.4.1

Fire protection systems and fire-fighting systems and appliances shall be maintained ready for use. PSCO observed on deck 4 zone 2 two cracks in drencher system. 74 SOLAS 20 II-2/14.1.1

Electrical installations shall be such that electrical services for safety will be ensured under various emergency conditions. PSCO observed extension cord installed in DC room through A60 boundary. 74 SOLAS 04 II-1/40.1.2

Fire protection systems and fire-fighting systems and appliances shall be maintained readt for use. PSCO's observed the following conditions in the fire "deluge" suppression system: 04 corroded nozzle pipe fittings, 02 missing clogged nozzles and 01 burst pipes on the cargo deck #03. Also observed 02 corroded nozzle fittings, 02 missing nozzles and 01 burst pipe on cargo deck #04

Rescue boats shall be stowed in a state of continuous readiness for launching in not more than 5 min. PSCO's observed starboard side rescue boat unable to be launched within the time required due to overfill of hydraulic oil in the davit motor.

The minimum fire integrity of all bulkheads and decks shall be as prescribed in tables 26.1 and 26.4 PSCO discovered multiple fire insulations in inadequate conditions in deck 4 in vicinity of frame 90 and 188. 74 SOLAS 81 Amend II-2/26.1

The hull, superstructures, structural bulkheads, decks, and deckhouses shall be constructed of steel or other equivalent material. PSCO discovered rusted steel deck in AC room #3 in vicinity of frame #188. 74 SOLAS 81 Amend II-2/23.1

Hinged fire doors in stairway enclosures, main vertical zone bulkheads and galley boundaries which are normally kept open shall be self closing and be capable of release from a central control station and from a position at the door. Fire door #801, 846, 845, 426, 427 did not close properly or did not latch when closing.

Plans and booklets hall provide information regarding fire protection, fire detection, and fire extinction. PSCO discovered inaccuracies in the Fire Control Plan (FCP). The following are not displayed properly: Locker #704 - plumber store (space category 10) and hotel manager office is labeled as locker (space category 7). Update FCP and space categorization.

13108 - The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. Port side steam pipe on forward bulkhead on ME space was actively leaking, estimated over 1 gallon per minute of water. 74 SOLAS (04) II-1 / 26.1 17C

07110 - In passenger ships carrying more than 36 passengers each machinery space of category A shall be provided with at least two suitable water fog applicators. High fog applicator on top of port side boiler did not work when activated locally. 74 SOLAS (01) II-2/7.6 17C

07115-Exhaust ducts from galley ranges in which grease or fat is likely to accumulate shall be fitted with a fire damper located in the lower end of the duct. PSCO discovered that duct on top of deep-fat fryer was caked with grease. 74 SOLAS 01 II-2/41-2.4.3 16C

Each special category space shall be fitted with an approved fixed-pressure water-spraying system for manual operation which shall protect all parts of any deck and vehicle platform, if any, in such space. During the re inspection of the drencher system we notice the following areas being deficient: Zone one has a flange that is leaking water. Zone two has one nozzle that blew out of the pipe, and five that were clogged. Zone three has one clogged nozzle. Zone Four has a crack pipe and a flange that is leaking water. Zone five had a nozzle that needs to be adjusted. Zone eight has one nozzle that blew out of the pipe, one nozzle that is clogged and one nozzle that needs to be adjusted. Zone nine has one nozzle that needs to be adjusted. Zone 10 has a flange that is leaking water. Zone 11 has two flanges that were leaking water. Zone 12 has one nozzle that blew out of the pipe. Zone 13 has two nozzles that need to be adjusted. Zone 15 has seven nozzles that are clogged.

Each special category space shall be fitted with an approved fixed pressure water-spraying system for manual operations which shall protect all parts of any deck and vehicle platform in such space. During testing of the drencher system in zone two 6 nozzles, zone three 2 nozzles, zone four 1 nozzle and flush value, and zone seven 1 nozzle were all clogged not allowing the flow of water. During testing of the drencher system in zones five and six, two ruptures were observed within the piping system limiting the effectiveness of the water spray pattern from the system. 74 SOLAS (97) II-2/37.1.3 17 ac

The company and the ship shall comply with the requirements of the International Safety Management Code. For the purpose of this regulation the requirements of the code shall be treated as mandatory. The company should establish procedures to ensure that the ship is maintained in accordance with the provisions of the relevant rules and regulations established by the company. Specific measures for identifying, and promoting the reliability of the vessel's fire suppression system are not adequate. An external audit is recommended.

Each special category space shall be fitted with an approved fixed-pressure water-spraying system for manual operation which shall protect all parts of any deck and vehicle platform, if any, in such space. During testing of the drencher system, PSC team discovered that the first three rows of fire zone one were inoperable, resulting in an inability to fight a fire in the associated sections of cargo deck 4. In zones 12 and 14, two ruptures were observed within the piping system, limiting the effectiveness of the water spray pattern and reducing the capability of the system. Also, during testing of the drencher system, the following nozzles were discovered to be clogged: Eight nozzles in zone two, three nozzles in zone three, 13 nozzles in zone four, one nozzle in zone five, two nozzles in zone six, two nozzles in zone eight, two nozzles in zone nine, one nozzle in zone ten, six nozzles in zone 11, three nozzles in zone 12, and five nozzles in zone 15.

PRECAUTIONS SHALL BE TAKEN TO PREVENT THE NOZZLES FROM BECOMING CLOGGED BY IMPURITIES IN THE WATER OR CORROSION OF PIPING, NOZZLES, VALVES AND PUMP. PSCO OBSERVED MUILTIPLE HI-FOG SPRINKLER HEADS ON DECK 08 THAT HAD CORROSION OBSCURING THE AMPULE AND NOZZLES. 74 SOLAS(01) II-2/10.7 16AC

FIRE DOORS IN MAIN VERTICAL ZONE BULKHEADS AND STAIRWAY ENCLOSURES, OTHER THAN POWER-OPERATED WATERTIGHT DOORS AND THOS WHICH ARE NORMALLY LOCKED, SHALL BE OF THE SELF CLOSING TYPE. THE SPEED OF THE DOOR SHALL IF NECESSARY, BE CONTROLLED SO AS TO PREVENT UNDUE DANGER TO PERSONS. FIRE SCREEN DOOR "M-618, M-623, M-813, M-702, M-706, AND M-702" WERE SLAMMING SHUT WITHIN SECONDS. 74 SOLAS 81 AMEND II-2/30.4 16AC

ACCOMMODATION AND SERVICE SPACES, STAIRWAY ENCLOSURES, AND CORRIDORS SHALL BE FITTED WITH AN AUTOMATIC SPRINKLER SYSTEM COMPLYING WITH THE REQUIREMENT OF REG 12. PSCO DISCOVERED MULTIPLE SPRINGLER HEADS IN THE GALLEY COVERED IN GREASE, THOSE SPRINKLER HEADS HAD DIFFERENT MODEL NUMBERS THAN LISTED IN THEIR HI-FOG MANUAL. 74SOLAS (01) II-2/41-2.5 16AC

MAINTENANCE, TESTING, AND INSPECTIONS OF LIFE-SAVING APPLIANCES SHALL BE CARRIED OUT BASED ON THE GUIDELINES DEVELOPED BY THE ORGANIZATION AND IN A MANNER HAVING DUE REGARD TO ENSURING RELIABILITY OF SUCK AND IN A MANNER HAVING DUE REGARD TO ENSURING REALIABILITY OF SUCH APPLIANCES. PSCO DISCOVERED AN EXHAUST LEAK ON #2 LIFEBOAT. 74SOLAS (20) III/20.3 16AC

MUSTER LIST AND EMERGENCY INSTRUCTIONS SHALL BE EXHIBITED IN CONSPICUOUS PLACES THROUGHOUT THE SHIP INCLUDING THE NAVIGATION BRIDGE, ENGINE-ROOM AND CREW ACCOMODATION SPACES. PSCO DISCOVERED THAT VESSEL'S MUSTER LIST THAT HAS NOT BEEN UPDATED TO REFLECT NEW FLAG SAFE MANNING CERTIFICATE REQUIREMENTS DATING 01JULY2020. 74SOLAS(20) III/8.3 10C

FIRE-FIGHTING SYSTEMS AND APPLIANCES SHALL BE KEPT IN GOOD WORKING ORDER AND READILY AVAILABLE FOR IMMEDIATE USE. PSCO OBSERVED A DOZEN DELUGE SPRINKER HEAD ON CARGO DECK 03 WERE CLOGGED AND UNABLE TO PROVIDE ADEQUATE COVERAGE. 74 SOLAS(20)II-2/14.2.1.2 10C

THERE SHALL BE A SUFFICIENT NUMBER OF CREW MEMBERS, WHO MAY BE DECK OFFICERS OR CERTIFIED PERSONS, ON BOARD FOR OPERATING THE SURVIVAL CRAFT AND LAUNCHING ARRANGEMENTS REQUIRED FOR ABANDONMENT BY TOTAL NUMBER OF PERSONS ON BOARD. PSCO DISCOVERED INCORRECT ASSIGNMENT OF LIFEBOAT CAPTAIN. 74SOLAS (20) III/10.3 10C

EXHAUST DUCTS FROM GALLEY RANGES IN WHICH GREASE OR FAT IS LIKELY TO ACCUMULATE SHALL BE FITTED WITH A FIRE DANPER LOCATED IN THE LOWER END OF THE DUCT. PSCO DISCOVERED THAT FD 03 LIGHT INDICATOR WAS NOT WORKING PROPERLY. 74SOLAS (01) II-2/41-2.4.3 10C

All ships of 3,000 gross tonnage and upwards shall be fitted with a 3 GHz radar or where consdiered appropriate by the Administration a second 9 GHz radar or other means, to determine and display the range and bearing of other surface craft, obstructions buoys, shorelines and navigational marks to assist in navigation and in collision avoidance. PSCO identifies vessel's S-Band radar was not in operation.

Fire fighting systems and appliances shall be kept in good working order and readily available for immediate use. RORO cargo deck drencher system piping was found wasted in the following zones causing water leakage from fractures: zones 2, 5, 6 and 13 (06 fractures on deck 4 and 01 on deck 3). Additionally, 02 drencher heads on deck 3 zone 8 were identified to be blocked.

Every crew member with assigned emergency duties shall be familiar with these duties before the voyage begins. MES team on starboard side MES was not knowledgeable of launching or emergency procedures. Crew was replaced and training conducted.

Lifeboats which are not self-righting when capsized shall have suitable handholds on the underside of the hull to enable persons to cling to the lifeboat. The handholds shall be fastened to the lifeboat in such a way that, when subjected to an impact sufficient to cause them to break away from the lifeboat, they break away without damaging the lifeboat. Four handhold mounting brackets on lifeboat #4 were missing, six mounting brackets found corroded on lifeboat #1, and eight found corroded on lifeboat #3.

The after staircase section valve located on deck 4 was found with combustible material loacted inside the space.

Fire door 706 was not closing properly. Fire door 803 was found without door handle.

Portable fire extinguisher dischare handle on deck 3 was found corroded and not in serviceable condition.

Exit doors leading from assembly station F to the embarkation deck were found locked. Door M-803 was found without a handle preventing excape from the galley to crew dining room.

Fire drills should be planned in such a way that due consideration is given to regular practice in the various emergencies that may occur depending on the type of ships and the cargo. Personnel on team boundary aft air regulator was not attached properly. Team enter the effected space without being on air. Training must be provided.

For the purposes of applying the definition of steel or other equivalent material as given in regulation 3.7 the applicable fire exposure shall be according to the integrity and insulation standards given in the tables of regulation 26 and 27. PSCO observed storage of combustible material in the shaft room which is a category 10 space.

THE CONSTRUCTION OF ALL DOORS AND DOOR FRAMES IN "A" CLASS DIVISIONS, WITH THE MEANS OF SECURING THEM WHEN CLOSED, SHALL PROVIDED RESISTANCE TO FIRE AS WELL AS TO THE PASSAGE OF SMOKE AND FLAME. PSCO OBSERVED MULTIPLE THROUHOLES ON DOOR 413 NEAR HANDLE LEADING TO GARAGE AREA #4.

EACH POWER-OPERATED SLIDING WATERTIGHT DOOR: SHALL BE PROVIDED WITH AN AUDIBLE ALARM, DISTINCT FROM ANY OTHER ALARM IN THE AREA, WHICH WILL SOUND WHENEVER THE DOOR IS CLOSED REMOTELY BY POWER AND WHICH SHALL SOUND FOR AT LEAST 5 s BUT NO MORE THAN 10 s BEFORE THE DOOR BEGINS TO MOVE. PSCO OBSERVED WATERTIGHT DOOR #5 CLOSING BEFORE ALARM SOUNDING.

THE CONSTRUCTION OF ALL DOORS AND DOOR FRAMES IN "A" CLASS DIVISIONS, WITH THE MEANS OF SECURING THEM WHEN CLOSED, SHALL PROVIDED RESISTANCE TO FIRE AS WELL AS TO THE PASSAGE OF SMOKE AND FLAME. PSCO OBSERVED FIRE DOOR M-800 NOT LATCHING CORRECTLY.

FIRE DOORS IN THE MAIN VERTICAL ZONE BULKHEADS AND STAIRWAY ENCLOSURES SHALL SATISFY THE FOLLOWING REQUIREMENTS: FIRE DOOR SHALL HAVE AN APPROXIMATELY UNIFORM RATE OF CLOSURE OF NO MORE THAN 40 s AND NO LESS THAN 10 s. PSCO OBSERVED FIRE SCREEN DOORS M-700 CLOSING TOO FAST.

EACH COMPLETED OPERATION SHALL BE SIGNED BY THE OFFICER OR OFFICERS IN CHARGE OF THE OPERATIONS CONCERNED AND EACH COMPLETED PAGE SHALL BE SIGNED BY THE MASTER OF SHIP. PSCO OBSERVED OIL RECORD BOOK HAS NOT BEING SIGNED BY THE MASTER SINCE JANUARY 13, 2022.

As an alternative a system meeting the requirment of regulation 37.1.3 may be fitted. Each special category space shall be fitted with an approved fixed pressure water spraying system for manual operation which shall protect all parts of any deck and vehicle platform in such space. Drencher system on deck 4 in zones 4,5,and 7 has 3 pipes that burst.

The bulkhead deck or a deck above it shall be weather tight. All openings in the exposed weather deck shall have coamings of ample height and strength and shall be provided with efficient means for expeditiously closing them weather tight. Eight of ten securing devices at the starboard side amidships embarkation door did not make contact with the wedges. Two securing devices on the top two corners were bent more than 20 degrees.

All arrangements used for pilot transfer arrangements shall efficiently fulfill their purpose of enabling pilots to embark and disembark safely. The appliances shall be kept clean, properly maintained and stowed and shall be regularly inspected to ensure that they are safe to use. One of the rope strands at the bottom of the port side pilot ladder was severed. Rope for the ladder was frayed on several locations and the bottom steps were deformed. The bottom three steps on the starboard pilot ladder were deformed, the second to last step was wooden when the others were rubber, and the ropes on the entire ladder were frayed.

Arrangements shall be provided to permit a rapid shut-down and effective closure of the ventilation system in case of fire, taking into account the weather and sea conditions. On the starboard side embarkation deck, six of twelve fire damper pull calbles were not labeled. On the port side embarkation deck, four out of eight fire damper pull cables were not labeled.

All life-saving applicances prescribed in this part shall be fitted with retro-reflective material where it will assist in detection and in accordance with recommendations of the Organization. The starboard side rescue boat was missing retro-reflective tape in some sections, and the tape in other sections was nearly absent.

Watertight decks, trunks, tunnels, duct keels and ventilators shall be of the same strength as water tight bulkheads at corresponding levels. A hole in deck 3 was discovered while testing drencher zone 14, leaking water directly on top of the starboard side steering pump controls.

The bulkhead deck or a deck above it shall be weather tight. All openings in the exposed weather deck shall have coamings of ample height and strength and shall be provided with efficient means for expeditiously closing them weather tight. Three of eight securing devices on the port side pilot door were missing a securing device or parts of the securing device.

Upon examination of the #4 life boat, the external shaft log had excessive vibration which caused the propeller blades to make contact with the rudder assembly housing and guard.

All access openings in bulkheads at ends of enclosed superstructures shall be fitted with doors of steel and secured with gaskets so as to make them weather tight. The A/C space door and ventilation dampers, and battery room door were missing gaskets.

Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. The Ro-Ro cargo deck drencher fire-fighting system had a total of two (02) nozzles that were clogged in cargo deck 3, zone 8, and one nozzle in Zone 10 that cannot be secured due to lack of thread in the pipe.

Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. The manual pulleys to open and close the Ro-Ro cargo spaces' natural ventilation were damaged and could not be operated. Ventilation covers are in the closed position.

Maintenance, testing, and inspections on fixed fire-extinguishing systems shall be carried out based on the guidelines developed by the organization in a manner having due regard to ensuring its reliability. PSCOs observed 5 missing drencher system nozzles covering the ro-ro cargo area. The nozzles were missing in the following zones: Two missing nozzles on Deck 3, Zone 9 and three missing nozzles on Deck 4, Zone 7. 74 SOLAS (20) II-2/14.2.2

The construction of all doors and door frames in "A" class divisions with the means of securing them when closed, shall provide resistance to fire as well as to the passage of smoke and flame, as far as practicable, equivalent to that of the bulkheads in which the doors are situated. PSCOs observed Deck 8, Door M-805, and Deck 7, Tropical Bar door to pantry with their lock sets removed leaving openings through the doors permitting a passage for smoke and flame. 74 SOLAS (97) II-2/30

Operational readiness, maintenance, and inspections shall be carried out in a manner having due regard to ensuring reliability of such appliances. PSCOs observed outer later of fuel hose between fuel tank and filter was dry rotted. 74 SOLAS (20) III/20

Inspection of life-saving appliances, including lifeboat equipment, shall be carried out monthly using the checklist required by regulation 36.1 to ensure that they are complete and in good order. PSCOs observed the number 4 lifeboat davit arm second sheave from the end squeaking due to lack of lubricant. During servicing on 07AUG2021 crew member assigned monthly servicing failed to report damaged grease fitting and was unable to grease sheave (Form CH 10/ 307). 74 SOLAS (20) III/20.3.1

The following fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs: structural fire protection, including fire- resisting divisions, and protection of operning and penetrations in these divisions. The PSCO fund the following A-60 fire doors blocked open; Deck 7, Door I-715 self closing device disconnected; Tropical Bar door to pantry blocked open by drainage cover, Deck 8, Door M-808 blocked open by dead bolt. 74 SOLAS (20) II-2/14.2.1.1

The maintenance plan shall include the emergency shutdown for fuel supply. The PSCO found during testing of the emergency generator quick closing valve that the stud holding one side of the closing mechanism was loose causing the excessive movement when the pull cable was pulled. The stud was tightened by the Chief Engineer and the valve was retested satisfactory. Upon further investigation, found that the quick closing valve was not included in the ship's maintenance plan.

The port side #4 lifeboat had a gap of at least 3 feet between the lifeboat and the embarkation station creating a safety hazard for the embarkation of the lifeboat.

Lifebuoy self-igniting lights required by 7.1.3 must be provided with a source of energy capable of meeting the requiremenst of paragraph 2.2 for a period of at least 2 hours. A total of 03 self-igniting lights (Deck 7 port and starboard, deck 8 port) were found inoperable. 74SOLAS(97) III / 31.2.3

Hydrostatic release untis shall be serviced at intervals not exceeding 12 months. Hydrostatic relerase unit of group of three liferafts located on the starboard side between lifeboats 1 and 3 was expired. 74SOLAS(97) III / 19.9.1

Maintenance, testing and inspections shall be carried out based on the Guidelines developed by the Organization and in a manner having due regard to ensuring the reliability of fire-fighting systems and appliances. The drench system in zones 2, 4, 7, 8 and 9 had multiple clogged nozzles.

Any other machinery space shall be adequately ventilated appropriate for the purpose of that machinery space. Mainswitch room (Door I-617) has no means of ventilation suitable for that particular machinery space

Every lifeboat shall have a food ration for every person it's certified to carry. There were numerous food rations throughout the lifeboats that had compromised vacuum seals. The vessel was found to have a total of 150 food rations to be replaced.

Foam fire-extinguishing systems; take samples from all foam concentrates carried on board and subject them to the periodical control tests in MSC.1/Circ.1312, for low expansion foam, or MSC/Circ.670 for high expansion foam. (Note: Except for non-alcohol resistant foam, the first test need not be conducted until 3 years after being supplied to the ship.) PSCO found the foam concentrate for the portable foam applicator in the engine room was manufactured in 4/2015 (4 years old); container had manufacturer's seal intact and had not been tested.

Electrical installations shall be such that the safety of passengers, crew and ship from electrical hazards will be ensured. The power outlet in the hotel directors office was overloaded by 2 power strips. In addition, a U.S. plug was connected to a European adapter with exposed ground creating an electrical fire hazard.

Where on any ship constructional features such as rubbing bands would prevent the implementation of any of these provisions special arrangements shall be made to the satisfaction of the Administration to ensure that persons are able to embark and disembark safely. PSCO found port and starboard pilot ladder were held from the side of the ship by two rubbing bands that prevent steps from resting firmly against the ships side.

The plan shall show the directions of escape and shall be properly oriented in relation to its position on the ship. The Mimic [name withheld] indicates secondary escape routes are segmented lines; however there are no secondary escapes indicated on the plan throughout the ship

Ships of all types should be subdivided into spaces by thermal and structural divisions having regard to the fire risks of the space. The Prisoners Area on Deck 6, between frames 147-153 (Cat 7) have been converted to a Beauty Parlour (Cat 2) and the Beauty Parlour (Cat 8) at frame 147 is currently A-0; bulkhead required to be insulated to A-15. Space not be used until A-15 insulation is installed.

After a safety equipment certificate survey has been completed no change shall be made without the sanction of the Administration. Lifeboat #4 was missing a cleat used for marshalling life rafts.

Atleast two lifelines shall be fitted to the davit spans. It was observed that one of the lifelines on lifeboat #4 was broken and operable.

Fire extinguishing appliances shall be kept in good order and available for immediate use at all times. Fire extinguisher 2020 was found to have been discharged. It was replaced with a spare one on board.

Electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in normal manner. The Public Address System located on the bridge was observed to smoke when operated during the drills.

Stowage and care of cylinders. Cylinders must always be secured and when not in use, they must be stowed in a rack in an upright position, with the valve cap protection cap in place. Propane were found loose on the deck.

The means of escape shall be marked by lighting or photoluminescent strips indicators. A missing section at the escape route at forward stair deck 7 was missing a section.

Fire doors in main vertical zone bulkheads and stairways enclosures shall be of the self-closing type. PSCO observed FSD 1801, M719 and I424 were not self-closing or were not displaying correctly on indicating panel.

Fire protection systems and fire-fighting systems and appliances shall be maintained ready for use. The pull cord for the fire damper above the [name withheld] cabin was inaccessible.

Objective evidence discovered during an expanded ISM exam revealed the following non-conformities. These discrepancies included with the other material deficiencies are evidence the crew/company are not meeting the SMS requirements. Recommend an external audit. 74 SOLAS 2014 IX/3.1 / ISM Code 1.3. See Form B for full details.

Ducts provided for ventilation to accommodation spaces shall not pass through ro-ro cargo spaces unless the ducts where they pass through a ro-ro space are constructed of steel in accordance with paragraphs 3.1.1. and 3.1.2. and are insulated to "A-60" standard within the ro-ro space. The insulation on the duct serving the escalators forward of fire door I412 and passing through the ro-ro cargo deck was damaged. The crew wrapped the duct for approximately 10 feet in material that was not "A-60" rated structural fire protection insulation. No nonconformity report was submitted, the crew already had spare "A-60" insulation onboard, and this condition compromised the A-60 fire class division as indicated on the Fire Control Plan. 74 SOLAS 1981 II-2/16.4.2.2.

Fire fighting appliances shall be kept in good working order and readily available for immediate use. Two portable fire extinguishers were observed without a charge, one had been moved from its bracket and both still had a seal indicating they had not been used. The relevant SMS monthly checklist indicated no nonconformities on 08OCT18. The galley fixed fire-fighting system controls were blocked by a heavy wheeled cart. On Deck 3, multiple fire fighting appliances were found blocked by two cargo containers during stevedore loading. On Deck 4 in the forward starboard side bosun store, a fire extinguisher and fire hose station were blocked by a ladder and miscellaneous stores. 74 SOLAS 2014 II-2/14.2.1.2.

All openings shall be provided with permanently attached means of closing. An A-60 door (I419) was blocked open with a plastic bottle and would not self-close with the bottle removed. In addition, the double leaf fire door to the galley (810) would not self-close. This has been a consistent issue for past exams. 74 SOLAS 1981 II-2/30.1.

Hoses specified in this Chapter as "fire hoses" shall together with any necessary fittings and tools be kept ready for use in conspicuous positions near the water service hydrants or connexions. Additionally in interior locations in passenger ships carrying more than 36 passengers fire hoses shall be connected to the hydrants at all times. Three fire hoses were found disconnected from hydrants: one in the ro-ro cargo deck (313) and two in the vicinity of the galley (616 and 805). 74 SOLAS 2014 II-2/4.7.1.

On all ships, for closed ro-ro spaces where fixed passage water-spraying systems are fitted, means shall be provided to protect the blockage of drainage arrangements. Two drains were missing gratings to prevent blockage and more than six grates were covered by large drain plugs, reducing the drainage capacity under signage stating clearly: "DRAIN OPENING. DO NOT COVER OR OBSTRUCT." 74 SOLAS 2014 II-2/20.6.1.5.

The purpose of this regulation is to contain a fire in the space of origin. For this purpose, the ship shall be subdivided by thermal insulation and structural boundaries; thermal insulation of boundaries shall have due regard to the fire risk of the space and adjacent; and the fire integrity of the divisions shall be maintained at openings and penetrations. There was missing segments of the thermal insulation in a penetration through A-60 bulkhead boundaries, compromising the fire protection in the following areas: 1. new laundry located on Deck 4 with missing thermal insulation in several areas and with penetrations that are not sealed, 2. Bulkhead adjacent fire door, number 402, on Deck 4, and 3. in several locations throughout the ro-ro cargo space where insulation had been damaged by moving cargo or the crew storing combustibles in small void spaces under stairs/tapered recesses. 74 SOLAS 2014 II-2/9.1

Fire fighting systems shall be kept in good working order. The piping for fire hydrant 901 had a fiberglass soft patch on the exterior by the dog kennel and the pipe was found wasted and wet on the other side of the bulkhead. 74 SOLAS 2014 II-2/14.2.1.2

Below the bulkhead deck two means of escape, at least one of which shall be independent of watertight doors, shall be provided from each watertight compartment or similarly restricted space or group of spaces. Two water coolers had been placed on the escape hatch – clearly marked to indicate what it was – from the engine room to Deck 4 near the stern ramp, obstructing one of the means of escape. 74 SOLAS 1981 II-2/28.1.1.

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. There are two life jacket storage closets in Muster Station D with thumbscrew locks that may allow a crewmember to secure access. During the abandon ship drill, the assembled crew were asked who had the key to the closets and none had one. 74 SOLAS 2014 III/20.1

Every crewmember with assigned emergency duties shall be familiar with these duties before the voyage begins. During the fire drill, on Deck 7, Main Vertical Zone 2, on the starboard side corridor, no crewmember checked cabins. On the port side corridor, one crewmember checked cabins by turning on lights and closing doors instead of the company policy to retrieve the toilet paper roll from the cabin water closet and place it on the door’s external handle. In addition, the stairway guide in the aft stairway, Deck 6, did not know the code for "fire" despite being onboard for three months. The Staff Captain witnessed this answer. 74 SOLAS 2014 III/19.2.1.

All life boats shall be equipped with sufficient tools and spares to conduct minor adjustments to the engine and its accessories. The tools on each lifeboat was different, inconsistent, and missing tools and spares in accordance with the life boat engine manufacturer's manual. 74 SOLAS 1997 III/41.8.27.

Survival craft and launching appliances shall be visually inspected to ensure that they are ready for use. The forward winch bowsing cable for the #1 Marine Evacuation System was worn (fish hooks over approximately 15cm) located at the first bend after the winch, last serviced 10OCT18 by a third party. 74 SOLAS 2014 III/20.6.1

Where necessary, non-conducting mats or grating shall be provided at the front and rear of the switchboards. Non-conducting mats were missing in front of the electrical panel servicing the anchoring gear. 74 SOLAS 2004 II-1/45.2.

Posters or signs shall be provided on or in the vicinity of survival craft. The operation instructions outside Marine Evacuation Systems 3 and 4 were posted in Greek. The working language of the ship in the official logbook is English and Spanish. 74 SOLAS 1997 III/9.2.

Cables and wiring shall be installed and supported in such a manner as to avoid chafing or other damages. Two portable extensions cords run from a power strip at the forward bulkhead and routed through the overhead. 74 SOLAS 1981 II-1/45.5.5.

Fire fighting systems shall be kept in good working order and readily available for immediate use. The drencher system was missing one nozzle in Zone 9. The last test of this system was conducted on 02OCT18. 74 SOLAS 2014 II-2/14.2.1.2.

Safe escape routes shall be provided and additional aids for escape shall be provided as necessary to ensure accessibility, clear marking, and adequate design for emergency situations. Desks and computers were installed in two designated store rooms to use as two makeshift offices (I426 and I405 storm rooms). There were no room-in-room alarms installed to alert crewmembers that there is a fire in the adjacent corridor. 74 SOLAS 2014 II-2/13.1

Survival craft operating instructions shall be provided at their vicinity and include the procedures of operation and deployment. The MES operating instructions on board the vessel were missing key steps necessary for its deployment such as the removal of safety pins

Every lifeboat shall be fitted with appropriate food rations for every person it is fitted to accommodate. There are a total of 17 needed on board.

Maintenance of lifesaving appliances shall be carried out on board. Lifeboat #2's forward sheave was making a squeaking sound when lowering the lifeboat and needed maintenance.

At all times during the voyage all lifesaving appliances shall be ready for immediate use. During the abandon ship drill the vessel's fast rescue boat would not start.

All fire extinguishers shall be of the approved type and design. Fire extinguishers on deck 6 #6026, 6015, extinguisher next to muster C life jacket stowage and deck 8 #8002 have brackets.

Every crew member assigned as a party member on a MES shall participate on a full deployment of such system at intervals specified in SOLAS. Crew members assigned as part of the MES have not participated in full deployment exercises.

Ships shall exhibit Fire Control Plans for the guidance of the ship's officers of the location where fire fighting equipment is located. The ship's Fire Control Plan lists a total of 15 fire fighting suits with their breathing apparatus. The vessel only has 12 operational breathing apparatus sets out of the 15 required.

Furniture in stairway enclosures shall be fixed. On deck 7, portside staircase there was loose furniture that could block the escape route.

The company should establish proved to ensure that the ship is maintained in conformity to the Regulations. The ship's SMS does not have clear or prescriptive procedures to verify that fire fighting suits and its protective equipment in fire stations is complete. The examination team noticed breathing apparatuses that are missing 1 mask, there were 2 empty bottles and another had a defective regulation valve.

Cylinders used for containing hazardous ships stores that are compressed gases must be stowed and secured appropriately. Refrigerant cylinders were found to be stowed directly to the deck.

Ships shall comply with the minimum fire integrity for bulkheads and decks as prescribed in tables 26.1 - 26.4. Combustible materials were found to be stored within a Category 10 space. (Deck 9 Fan Room #1) and has A/C united 1 and 2.

Code 0720-All nozzles shall be of an approved dual-purpose type (i.e. spray/jet type) incorporating a shutoff. A PSCO identified in fire station #307 the nozzle for the fire hose was missing. 74SOLAS(97)II-2/8.4.

Code 0960-Escape routes shall not be obstructed by furniture and other obstructions. A PSCO found an escape route being obstructed by wood planks by door # I-418. 74SOLAS(97)II-2/28-1.3

Code 0745-After any survey of the ship under regulations 7, 8, 9 or 10 has been completed, no change shall be made in the structural arrangements covered by the survey, without satisfaction of the Administration. A PSCO found that the closing devices were missing on fire dampers #10, #11, #20 and #6. 74SOLAS(97) I/11(b).

Code 0750-The construction of all doors and door frames in "A" class divisions with the means of securing them when closed, shall be provide resistance to the fire as well as to the passage of smoke and flame. A PSCO found the following doors not lashing to close position: I-119, I-410, 407, 406, 708, M-711. 74SOLA(97)II-2/30.2

Code 0630-Maintenance and inspection of life-saving appliances shall be carried out in a manner having due regard to ensure reliability of such appliances. The PSCO found #4 lifeboat davit winch making excessive noise when being lowered and the brake being applied. 74SOLAS(14)III/20.3.1.

Code 0630-Maintenance and inspection of life-saving appliances shall be carried out in a manner having due regard to ensure reliability of such appliances. The Recognized Organization issued a Short-Term Passenger Ship Safety Certificate reducing passenger count to 900 to expire 20MAY2018 due to lifeboat #4 and #3 removed from service as required by Flag State. 74SOLAS(14)III/20.3.1. (16.a.c.)

All lifesaving appliances shall be in working order. PSCO found the following: 1. #4 lifeboat wheel was frozen on the forward davit arm bottom wheel. 2. #2 lifeboat the forward top shive on hte davit arm was excessively squeaking due to lack of lubrication. 0630, SOLAS (14)III/20.2

An administration may allow a seafarer to serve for a period not exceeding three months onboard a ships entitled to fly its flag while holding an appropriate and valid certificate as required by another party, but which has not yet been endorsed so as to render it appropriate for service onboard ships to fly the flag of the administration. PSCO found the Ship's Security Officer had been onboard for seven months without holding a STCW I/10 endorsement. 0220, STCW I/10.5

Doors required to be self closing shall not be fitted with holdbacks. Their were several Fire Screen Doors on deck #4 with door wedges holding the self-closing door open. 0710, SOLAS (97) II-2/47.2

Escape routes shall not be obstructed by furniture and other obstruction. Several corridors were obstructed by garbage cans. 0960, SOLAS 97 II-2/28-1.1.3

Detectors and manually operated call points shall be grouped into sections. The activation of any detector or manually operated call pint shall initiate a visual and audible fire signal at the control panel and indicating unit. The PSCO found the call point missing in way of Fire Screen Door 618. 0730, SOLAS (97) II-2/12.1.4

Fire-fighting systems and appliances shall be kept in good working order. Multiple penetrations in drencher piping was observed during the test of the systems in way of zone b on deck 4, between frames 33 and 45. 0725, SOLAS (14) II-2/14.2.1.2

At all times while the ship is in service, the requirements of paragraph 1.1 shall be complied with. PSC found the following decripencies: (a) Hose box 603 - atching device broken. (b) Hose box 611 - Missing nozzle. (c) Hose box 609 - not fitted with a dual purpose nozzle.

Only public toilets, lifts, lockers of non-combustible materials providing storage for fafety equipments and open informatation may be located withing the stairway enclosure boundries. PSCO found an excessive amoint of combustible materials stored at the reception dest on deck #6. Storage shall be limited to combustible materials that can be consumed in a day. 0750, 16c.

The construction of all doors in "A" class divisions fitted with the means of securing tehem when closed. The latching device on aft door Deck #8, Beverage Store, was inoperable.

The construciton of all doors and door frames in "A" class devisions shall provide resistance to fire as well as the passage of smoke and flames. PSCO found that Fire Screen Door 805 was not closing.

Inspection of he life-saving equipment shall be carried out monthly using hte checklist required by regulation 36.1 to ensure that they are completed and in good order. PSCO found that the bottom forward wheel on he #4 lifeboat davit was not rotating.

The ILLC shall not be delivered to the ship until the officer or surveyor has certified that the marks are correctly and permanently indicated on the ship's sides. The side of the ship has a Summer Load Line and the certificate does not. ICLL 66/88, Annex I / Reg. 9, 50ac

The master of a ship shall comply with the requirements of adopted ship reporting systems. There is no acknowledgement of the GMDSS test message. 74 SOLAS (14), Ch. V / Reg. 11.7, 16ac.

A portable rechargeable battery operated lamp shall be provided unless supplementary emergency lighting, as required by subparagraph .1, is provided. The emergency supplemental lighting on Deck 5 starboard crew corridor is not working. 74 SOLAS (97), Ch. II-1 / Reg. 42-1.2, 16ac

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations. There is a strong smell of diesel in Lifeboat 4 and a visible sheen in the bilge suggesting a fuel leak. In addition, rain water was observed dripping from compromised helm window seal contributing to approximately 30 gallons of water in the bilge. 74 SOLAS (14), Ch. 1 / 11(a)
