There were three different sets of Periodic Safety Test procedures in the Engine Control Room, which had different document numbers; one was for a different vessel that was actively being used to test equipment, two did not match the MSC letter/document reference number listed on Marine Safety Center’s letter indicating the PSTPs were ready for testing. No PSTPs were provided that included MSC’s approval letter or indicated final approval. Furthermore, no documentation could be provided showing the PSTPs were tested annually.

The stbd lifeboat's onboard brake release cable is kinked and does not function as designed. The cable fell off of the spool during lowering and pulled entirely out of the lifeboat, prohibiting manual brake release from inside the boat. Additionally, the crew stated this had been ongoing for over a month; deficiency was not reported as per 10.2.2 of the ISM Code.
