The crew failed to follow procedures related to the bilge suction valves post maintenance by ensuring they were open in case of flooding. An internal audit of procedures is recommended.

An efficient bilge pumping system shall be provided, capable of pumping from and draining any watertight compartment...under all practical conditions. The bilge suction valves in the pump room as well as both in the forward bosun locker were all closed. Contrary to their procedures requiring them to be oepn. Crew opened all valves.

Shal be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space concerned in the event of a fire occuring in the space which such tanks are situated. Quick closing vavle for the emergency generator fuel tank not capable of fully closing the tank valve from outside the space.
