Testing and calibration of fixed gas detector should be carried out at regular intervals and suitable equipment and span gas for this purpose should be carried on board. Calibration testing is not being conducted in accordance with cargo operations manual which refers to manufacturer manual and suitable testing equipment was not on board. Manufacture manual requires testing of fixed gas detection equipment to be done monthly. Last calibration test was conducted in August 2017. During testing PSCO witnessed that calibration gas onboard was not in accordance with the cargo operations manual's prescibed requirement for 30% LEL propane mixture with balance of nitrogen. In addition, the span gas was empty. Action taken: 17 A,C

Resolution: Vessel updated procedures and obtained new bottles of span gas.
Emergency shutdown valves should fully close under all service conditions with 30 seconds of actuation. During testing of all cargo and vapor piping ESD valves PSCO witnessed valves close in excess of 40 seconds. Action take: 10 C
