The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull, protection against fire, means of escape in case of a sudden unexpected casualty, guards and rails in hazardous places, ventilation of enclosed spaces, and necessary facilities for passengers and crew. At all inspections, special attention shall be paid to insure that guards or protect devices are installed in all hazardous places. Guard on generator observed to be loose. 46 CFR 176.25-10(b) Old T

Flexible or portable cords cannot be used for permanent wiring installation. Remove all extension cords throughout the bridge area. 46 CFR 183.10-20 Old T

At each subsequent inspection for certification and re-inspection the marine inspector shall inspect all lifesaving equipment and determine its serviceability. Life float light bracket observed to be broken. 46 CFR 176.25-20(a) Old T

A vessel must comply with the requirements for any radio installations, including the requirements for a station license and installation certificates issued by the FCC, in 47 CFR part 80. GPS location not tied into radio. 46 CFR 184.502

Scantlings, material, fasteners, workmanship, and arrangements of each vessel as to provide for safe operation. Fire detection must be installed such that the actuation of alarm in one area results in both audible and visual alarms in all areas. Vessel did not have integrated fire detection. 46 CFR 185.405(C) Old T

Electrical installations and equipment shall be installed to prevent fire caused by electrical equipment and safe from shock of personnel. Remove all dead end wires throughout the vessel. 46 CFR 183.01-5(a) Old T

Lighting fixture in steering space was missing bulb and housing causing the fixture to become corroded beyond serviceable condition.

Starboard rudder post was missing 2 nuts on the shaft packing due to vibration. system also had numerous hydraulic leaks.

Aft fuel tank had considerable pitting > 15%. tank must be removed from service until repairs can be made or tank is permanently removed from service.

Generator had exposed moving parts with no guard and was missing lagging.

"Adult life jacket must be carried for each person on board." 11 life jackets were on board vessel while the COI permits up to 13 total persons. Vessel rep provided proof of purchase of the life jackets after the conclusion of inspection. Deficiency cleared.

"A weathertight door must be provided for each opening located in a deck house." The weathertight door on the port entrance to the deck house was missing its gasket on the forward vertical portion. Gasket shall be provided to ensure it is made weathertight.

"At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of machinery, fuel, and piping systems." Wiring penetration was loose on the fwd bulkhead port and starboard side. Ensure the fittings are watertight and not loose. Sump hose under generator was inoperable due to chaffing. Sump hose shall be replaced. Starboard fwd section of raw water cooling leading to wet exhaust was corroded and requires replacement. Aft transverse section of bilge piping in steering compartment was wasted and had a broken bracket; installation requires piping renewal and reinstalled bracket.

Except as provided by paragraph (d) of this section, each life jacket must be approved in accordance with either 160.002, 160.005, or 160.055 in subchapter Q of this chapter, or other standard specificed by the Commandant; 02 life jackets were significantly deteroriated; Replace 02 life jackets by 03JAN20 as per 46 CFR 180.71(c); To the satisfaction of the Coast Guard.

Except as otherwise provided in this section, rails or equivalent protection must be installed near the periphery of all decks of a vessel accesible to passengers of crew; Vessel did not have adequate protection on back of the main deck; Install adequate protection by 03JAN20 as per 46 CFR 177.900(a); To the satisfaction of the Coast Guard.

Piping, including valves, pipe fittings and flanges, conveying vaport, gas, or liquid, the temeprature of which exceeds 65.5C (150F). must be suitably insulated where necessary to prevent injuries; Lagging was not present on all components of propulsion main engine exhaust; Apply sufficient lagging on engine exhaust by 03JAN20 as per 46 CFR 177.970; To the satisfaction of the Coast Guard.

Each battery must be located as high above the bilge as practicable, secured to protect against shifting with roll and pitch of the vessel, and free from exposure to water splash or spray; Batteries had excessive foreign debris on them; Clear battery surface from excessive debris by 03JAN20 as per 46 CFR 183.350(b); To the satisfaction of the Coast Guard.

As appropriate for the intended voyage, a vessel must carry adequate up to date charts of large enough scale to make safe navigation possible; Paper charts were not corrected to the most recent NTM and were over a year oldl Acqure corrected charts by 03JAN20 as per 46 CFR 184.420(a); To the satisfaction of the Coast Guard.

Port rudder had severe cracking on support welds. Additional cracking was identified in the located on deck support welds. Provide proper repairs to welding around the failed welds on both shafts.

Vessel call sign on station liscence did not match call sign on safety radio certificate. requried vessel rep to provide safety radio cert with ammended call sign. (note FCC safety radio cert is not availible as a def in misle therefore I put it under the CSSR cert. that cert is not required onboard the vessel)

vessel master did not have a radio liscence, vessel provided an alternate master witha liscence in order not to no sail the vessel required the vessel to provided a radio liscence for that master within 7 days and prior to him operating the vessel.

two penetrations in the superstructure were not properly sealed. required vessel rep to "provide weathertight wire and A fittings on the superstructure."

vessel's EAP drug and alcohol phone number did not work. required vesel rep to provide a valid employee assistance program.

Crew failed the first two drills onboard, "crew training was insufficent for required drills"

Provide fuel piping of proper material IAW 46CFR182.455 and 182.20-40 Prior to Sail

Provide renewal of wasted bilge piping as marked by marine inspector IAW46CFR182.510 and 182.40-5 Prior to Sail

Provide electrical wire supports IAW46CFR183.10-5 by 05DEC15.

Provide inserts to locations marked by marine inspector to good metal; inserts must be NDT and verified by Marine Inspector

Make permanent proper repairs to four cracked welds indicated on the aft bulkhead in the lazarette.

Gouge and reweld indicated repair sections in the engine room.

Make permanent proper repairs as indicated in the collision void.

Forepeak wiring needs to be in an enclosure. Wiring penetrating through bulkhead needs to be sealed.

The painter was coiled into a pvc tube and did not release freely.

The bracket attaching the "jockey bar" to the rudder post was severely wasted.

The aft deck had deck rail with a broken weld at the base on the starboard side forward.

The engine compartment overhead on the starboard side forward had a 1.5 to 2 inch hole in the deck plating.

The port engine exhaust exit in the rudder compartment had a cracked weld.

Epoxy was used to make temporary repairs on the engine exhaust (port) aft of the water injection point.

The aft pilot house window had a leaking gasket.

The water intake tubing was cracked.

The fuel manifold (e/r forward) had piping without blanks on the open end in addition to open piping for the spare fuel storage tank in the stern compartment.

The rescue platform was not designed to properly work with the vessel design. No rescue harness was present.

EPIRB battery expired. Hydrostatic release expired.
