02108 - A gas carrier shall comply with the requirements of the IGC Code. Electrical equipment, cables and wiring should not be installed in hazardous locations unless it conforms with the standards not inferior to those acceptable to the Organization. PSCO observed IGG/GCU Inlet and Accumulator High Level Temperature Sensors located in compressor room had improperly packed housing, allowing for accumulation of flammable vapors. - 74SOLAS(20) VII/13.1; IGC Code(93)/10.1.4 - 10c

The condition of the ship and its equipment should be maintained to conform with the provisions of the code to ensure that the ship will remain fit to proceed to sea without danger to the ship or persons on board. Several cargo leaks were discovered o the vessels number 3 and number 4 vapor domes. Inspectors noted visual and audible and meter-indicated evidence of leaks.

Whenever an accident occurs to a ship or a defect is discovered, either of which affects the sfety of the ship or the efficiency or completeness of its life saving appliances or other equipment, the master, or owner of the ship should report at the earliest opportunity to the Administration, the nominated surveyor or recognized organization responsible for issuing relevent certificate. Vessel and company failed to nitify the Administration or Recognized Organization or Nominated Surveyor when cargo leaks were discovered on number 3 and number 4 tank vapor domes.

Whenever there is a hazardous condition either on board a vessel or caused by a vessel or its operation, the owner, agent, master, operator, or person in charge must immediatley notify the nearest Coast Guard Sector Office or Group Office and in addiditon submit and report required by 46CFR4.05-10. Coast Guard was never notified about the vapor dome leaks by the owner, agent, master , operator, or person in charge.
