The master shall conduct sufficient drills and give sufficient instructions to make sure that all crew members are familiar with their duties during emergencies that necessitate abandoning ship or the recovery of persons who have fallen overboard. Man overboard drills were conducted during inspection. MI determined that crew member was not familiar with duties during man overboard drill. Crew shall conduct sufficient drills and demonstrate familiarization to the satisfaction of the attending marine inspector.

The master shall conduct sufficient fire drills to make sure that each crew member is familiar with his or her duties in case of a fire. Each fire drill must include: Summoning the crew to report to assigned stations and to prepare for and demonstrate assigned duties; Instruction in the use and location of fire alarms, extinguishers, and any other fire fighting equipment on board.(c) Each fire drill must, as far as practicable, be conducted as if there were an actual emergency.

Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. All closure devices must be readily available and mounted in the vicinity of the vent. During operational test of the machinery space ventilation closures, the stbd side closures did not make a proper seal. Make necessary corrections to ensure a proper seal by 17Nov2025.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. MI noted hydraulic hose fittings on stbd sided of bow ramp displayed corrosion and wastage. Replace hose end fittings.

Provide status of replacement ramp pin bushings and estimated installation date.

Replace stainless steel ramp pins as mentioned by vsl captain.

Add extra pin brackets on ramp as mentioned by vsl captain.

Main engine room port side ventilation cover remote pull cable was not fully releasing.

Port main engine shaft guard was removed and not guarding the shaft. Re-install guard.

Ship's whistle inoperable.

Public address system was did not have complete audible coverage on the forward car deck.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. Repairs or alterations that affect the safety of the vessel include, but are not limited to: replacement, repair, or refastening of deck or hull planking, plating, and structural members; repair of plate or frame cracks; damage repair or replacement, other than replacement in kind. MSD St. [name withheld]/IO's discovered that the cargo loading ramp hydraulic ram broke free from the weld preventing the ramp from being closed prior to sailing from Red Hook to St. [name withheld].

Stbd gen exhaust leak & lagging. Remove lagging to inspect for leak, crack, etc… Replace lagging after repairs are made

Ships whistle is not working properly

Replace all cracked wooden plugs on natural ventilation gooseneck vents.

Main engine room ventilation cover was missing from the vent located above the starboard side of the house. Remote pull cable was also missing.

High level aft bilge alarm visual indicator for the rudder room located at the operating station is inoperable. Until proper repairs are made to the visual indicator, vessel is required to log a visual inspection to verify water ingress prior to every departure from the dock. Log must include date, time, name and signature of the master.

Provide battery switches between starting batteries and each of the port and starboard generators.

Storage locker located stbd side of the house at main deck has a broken dog on weather hatch.

Main deck does not have any markings to indicate where lifesaving equipment including life jackets are stowed. Provide markings in conspicuous locations on both sides of the vehicle deck.

Starboard forward lifering buoy floating light is inoperable.

Port side main deck fire station valve bent. During operational test of fire hoses, water leak was observed from the valve.

Vessel Public Address system cannot be heard from all required spaces.

Aft starboard fire extinguisher bracket wasted and cannot properly support extinguisher.

Lifefloats deteriorated from weathering. Replace lifefloats or remove them from the vessel. Provide proper weak-link to replace installed hydrostatic release.

Forward masthead steaming light inoperable.

Fire drill, abandon ship and man-overboard drills have not been logged. Provide log entries in the logbook.

Provide missing charts and nautical publications.

Operator did not conduct annual inspection as required. Last inspection was in 2018.

Onboard crew and posted instructions for operation of fixed gas firefighting system unclear as to actions to take and order of actions to discharge system. Provide crew training and revise posted instructions to reflect manufacturer recommendations for system operation.
