Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant [name withheld] a grounding that caused damage to the port and starboard hulls, the vessel's operator was instructed to submit a repair proposal prior to commencing work. Repairs were subsequently completed without an approved plan and without Coast Guard oversight. The vessel is detained until the following conditions are met: - A formal repair plan is submitted to and approved by the OCMI, and - The approved repairs are completed with direct oversight from local Coast Guard Marine Inspectors. {46 CFR 176.700(a)}

Vessel sustained damage while running aground. Provide a plan for permenent repairs to MSU Sturgeon Bay prior to starting repairs.

A fire pump may be driven by a propulsion engine. A fire pump must be permanently connected to the fire main and may be connected to the bilge system. Vessel's crew was unaware how to operate the fire pump to achieve the required pressure. Off duty crew member was recalled and able to provide training during the inspection; however, on duty crew was unfamiliar with the system. Company to ensure training is conducted with all crew members for vital system operations. Marine inspector will reattend to verify crew member's training with the system.

A catamaran vessel must be equipped with bilge pumps for each hull, as if each hull is a separate vessel. Vessel's crew was unaware how to operate the fix bilge system, or the cross over system which connects the hulls. Off duty crew member was recalled and able to provide training during the inspection; however, on duty crew was unfamiliar with the system. Company to ensure training is conducted with all crew members for vital system operations. Marine inspector will reattend to verify crew member's training with the system.

A vessel must carry one floating water light attached to the ring life buoy. Floating water light on ring life buoy was inoperable. Conduct necessary repairs.

The master shall conduct sufficient drills and give sufficient instructions to make sure that all crew members are familiar with their duties during emergencies. Some crew members were unfamiliar with abandon ship procedures as this type of drill has not been conducted recently. Conduct sufficient drills and record as appropriate for all crew members.

Fire fighting, abandon ship, and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. Training records demonstrating sufficient instruction was provided to the crew was not available. Ensure monthly drills are conducted, and the logs capture the date of the drill and training and general description of the drill scenario and training topics.

For credentialed mariners, the general medical exam must be documented and of such scope to ensure that there are no conditions that pose significant risk of sudden incapacitation or debilitating complication. Master of the vessel had an expired medical certificate. Provide crew with valid medical certificate.

A vessel must be provided with a satisfactory arrangement for draining any watertight compartment. Installed electric bilge pumps in various compartments are inoperable. Conduct necessary repairs.

A vessel must comply with the applicable requirements for any radio installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR 80. Vessel lacks required FCC Certs.

The inspection and maintenance of survival craft shall be logged or otherwise documented for review by the Coast Guard upon request. Inflatable life raft maintenance records not available.

Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10, to include monthly inspections. Vessel unable to provide evidence of monthly testing. Ensure inspections are conducted and recorded.

The vessel must carry adequate and up-to-date charts to make safe navigation possible. Vessel's digital charts have not been updated for an unknown period of time. Provide updates per the manufacturer's recommendations at specified update intervals.

Each life ring life buoy must be marked in clearly legible block capital letters with the vessel's name.

Chemical testing of personnel must be conducted and in accordance with the procedures detailed in 49 CFR part 40. Vessel was unable to show proof of employee enrollment in a random chemical testing program.

Vessel's COD on board was expired.

Aspririn was expired in first aid kit.

Vessel was short one type I pfd for full COI complement.

Lifejacket stowage compartments had improper number of pfds stowed.

Observed both engine room fire Boys due for servicing. Both indicators in Green Zone. Both Fixed extinguishers to be service and report provided to USCG Marine Inspector on or before 08JULY2023 as required by 46 CFR 176.810

Observed both engine room hatch cover gaskets not fully intact and portion missing. Replace gaskets on both engine hatch covers as required by 46 CFR 179.360. To be resolved on or before 08JULY2023.

EAP Hotline Number as required by 46 CFR 16.401 missing. Post Hotline Number.

List of employees covered as required by 46 CFR Part 16 not current. Provide current list to CG MI.

Vessel needs to provide copy of updated COD. 46CFR67.313

First aid kits shall comply with the requirements/contents of 46 CFR 160.041. Items in first aid kit were expired; replace as needed. 46 CFR 184.710

Ring life buoys shall comply with requirements of 46 CFR 185.604 (markings). Ring life buoys need to be re-stenciled with vessel's name. 46 CFR 185.604

Each life raft must be stowed as required by 46 CFR 180.137. Painter line was not attached properly. Crew fixed configuration on the spot.

Vessel/company has not submitted annual drug testing results (MIS reports) to the CG for 2016 and 2017. MIS reports are to be submitted each year by 15 March. Submit all missing MIS reports to CG.

Employee Assistance Program information must be posted and available to employees. Post contact information for EAP resources.

Company does not have a sufficiently detailed drug testing program to encompass all required components IAW regulations. Establish/document a proper drug testing program/policy.

The extra life ring installed on the back deck must be labeled with vessel name.

The Inflatable Buoyant Apparatus installation does not have launching instructions posted at the installation. Post permanent instructions near the installation.

An updated chart for operating area must be on board and maintained in updated condition at all times.

Hoses on stbd main engine must be provided w/chaffing gear to prevent damage to flexible hoses.

Each day prior to getting underway w/passengers the master of the vessel shall require the crew to perform all drills to his/her satisfaction. Within 7 days of the issuance of this 835 the owner/operator shall arrange for CG inspection so that CG inspectors can witness drills to ensure adequacy and improvement in crew's capabilities.

Vessels pre-engineered gas fire extinguishing system does not have a helm display at the operating station for both fireboys. A second helm display must be installed as per the manufactures instructions. 46 CFR 181.420

Fire hose was leaking water at the nozzle connection end. Replace hose and show proper operation to the attending CGMI. 46 CFR 181.320

Installed fuses are not of the cartridge type. Replace fuses with cartridge type. 46 CFR 183.380 (l)

Vessel's engine room compartments do not provide a fire boundry due to having cable penetrations not properly sealed. Properly fill in void with fire barrier sealant. 46 CFR 177.410

Vessels upper deck is not marked with maximum passengers as indicated on the vessels stability letter. Properly mark the upper deck with max passengers with a durable marking of at least one inch.
