Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant [name withheld] a grounding that caused damage to the port and starboard hulls, the vessel's operator was instructed to submit a repair proposal prior to commencing work. Repairs were subsequently completed without an approved plan and without Coast Guard oversight. The vessel is detained until the following conditions are met: - A formal repair plan is submitted to and approved by the OCMI, and - The approved repairs are completed with direct oversight from local Coast Guard Marine Inspectors. {46 CFR 176.700(a)}

A fire pump may be driven by a propulsion engine. A fire pump must be permanently connected to the fire main and may be connected to the bilge system. Vessel's crew was unaware how to operate the fire pump to achieve the required pressure. Off duty crew member was recalled and able to provide training during the inspection; however, on duty crew was unfamiliar with the system. Company to ensure training is conducted with all crew members for vital system operations. Marine inspector will reattend to verify crew member's training with the system.
