The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. PSCO observed active hydraulic fluid leak on cargo hatch cover control box 2 on port side. 74SOLAS(20) I/11(a)

During the lifeboat operational test, the rudder was stuck for 35 minutes. During inspection, the PSCO noticed the citadel leading into the steering gear room was locked from inside not allowing the watertight door to close. 74 SOLAS 14 XI/8.2

Means shall be provided to control leaks of flammable liquids. The F/O drain tank pipe had a modified flange affixed to a "T" in the pipe in way of the fwd, stbd overhead on the bottom deck of the engine room. Oil soaked lagging was observed in the area of modified flange. 74 SOLAS 14 II-2/4.1.1

If the vessel's gyrocompass stop operating properly, the person directing the movement of the vessel must report or cause to be reported that is not operating properly to the nearest COTP. The ship's gyrocompass has been malfunctioning since 07MAR2019. Vessel arrived in Sector Houston Galveston on 14 MAR2019. Vessel did not notify COTP.

The company should establish the procedures plans and instructions for key shipboard operation concerning the safety of the personnel, ship and protection if the environment. The various tasks should be defined and assigned to qualified personnel. Ship's pre-arrival check list requires crew inspection of gyrocompass and the radio logbook instructions address recording monthly EPIRB check. Gyrocompass was faulty but was checked as satisfactory on pre-arrival checks performed by the crew were not being recorded in the radio log book. The above observations and deficiencies 1, 2 and 4 are objective evidence of the lack of effectiveness to the implementation of the ISM code and an internal audit is required with a focus on record keeping and reporting. 74 SOLAS 14 IX/ 3.1. Maintenance, testing, and inspections shall be carried out based on the guidelines developed by the organization and in a manner having due regard to ensuring the reliability of fire-fighting systems and appliances. PSCO witnessed an error status on the oil myst detector and subsequently varified no maintenance had been completed in accordance with the manufacturer's manual.
