A minimum of two two-way portable radiotelephone apparatus for each fire party for fire-fighter's communication shall be carried on board. No radios are provided on board for either fire team. Resulting in the fire-fighter entering the space with no means of communication.

A fixed fire detection and fire alarm system shall be of an approved type and comply with the FSS code. Heat detectors shall be certified to operate before temps exceed 78 degrees Celsius but not until the temperature exceeds 54 degrees Celsius. The crew has no means of ensuring the heat detectors are being tested within the parameters of the FSS code as vessel is currently using a lighter to test heat detectors. Additionally suitable instructions and component spares for testing and maintenance shall be provided; vessel has no written instructions for the testing of heat detectors.

All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in the normal manner. Ship's force supplied power to energize pump on the pier via pigtail plugged into a outlet using only electrical tape. Recommend using solid cable or utilize proper electrical connections to reduce hazard to personnel onboard.

Resolution: Installation removed from service. Encouraged crew to find a more permanent solution, safer for crew onboard.
