It was reported that the VECS oxygen sensor was malfunctioning resulting in the VECS oxygen analyzer system being inoperable and unavailable for use as intended. MARPOL VI/15.5

..oil tankers... shall be provided with a fixed hydrocarbon gas detection system complying with the Fire Safety Systems Code for measuring hydrocarbon gas concentrations in all ballast tanks and void spaces of double-hull and double-bottom spaces adjacent to the cargo tanks, including the forepeak tank and any other tanks and spaces under the bulkhead deck adjacent to cargo tanks. It was reported that the fixed gas detection system for multiple ballast water tanks was taken out of service and rendered inoperable. 74 SOLAS (20) II-1/4.5.7.3.1

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. Free fall lifeboat sustained damage on the starboard quarter in way of the conning position during lifeboat drill on 28OCT2019. Crew made temporary repairs to the internal and external fiberglass components. Vessel to provide satisfactory technician report or alternative arrangements to class, flag and USCG satisfaction prior to departure.

The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. In meeting these requirements the company should ensure that: any non-conformity is reported with its possible cause, if know; appropriate corrective action is taken; and records of these activities are maintained. As outlined in deficiency #1, the free fall lifeboat sustained damage on 28OCT2019. The master informed the company via email of damage to the sprinkler system but did not inform them of structural damage to the lifeboat. Furthermore, reports to the Coast Guard and classification society did not mention damage and temporary repairs to the fiberglass. Initial examination of lifeboat by PSCO revealed that the temporary repairs were painted over and covered with reflective tape. No repair plan or proposals were found on board. Objective evidence discovered in an expanded ISM exam revealed that the vessel failed to fully implement the requirements of the SMS as evident by deficiency #1. An internal audit is required within in 3 months.
