Gas carriers carrying cargoes emitting toxic vapours or gases* shall carry lifeboats with a self-contained air support system complying with the requirements of section 4.8 of the Code. A lifeboat with a self-contained air support system shall be so arranged that the air in the lifeboat remains safe and breathable and the engine runs normally for a period of notless than 10 min and pressure of atmosphere inside shall not exceed outside atmosphere pressure by 20 hPa. Solas74(2014) III/31.1.6, LSA Code CH IV, 4.8 PSCO's observed air supply hose to be bleeding air directly into lifeboat.

Gas carriers carrying cargoes emitting toxic vapours or gases equipped with self-contained air support systems shall remain safe and breathable and runs for a period of not less than 10 mins. Self-contained air support system was leaking air and dropping steady pressure.

Ships shall carry one or more totally enclosed lifeboats complying with 4.6 of the code. Each lifeboat shall be arranged that access to the lifeboat is provided with hatches which can be closed to make the lifeboat watertight. Hatchway gasket material was found unsealed and falling off the hatch entrance.

Gas detection equipment should be so designed that it may readily be tested. Testing and calibration should be carried out at regular intervals. Fixed gas calibration certificate provided to PSCOs was from construction yard, dated 26OCT2016. During test of fixed gas detection system, 50% LEL propane used, PSCOs witnessed reading of 63%. In calibration mode, PSCOs witnessed reading of 166%. 93 IGC 13.6.6 17ac

Maintenance, testing and inspections shall be carried out based on the guidelines according to the Organization* in a manner having due regard to ensuring the reliability of fire-fighting systems and appliances. (MSC.1/Circ.1432) Annual inspections should be carried out to ensure that the indicated actions are taken for the specific equipment: (7.9) Fixed dry powder systems. Vessel’s most recent annual inspection of fixed dry chemical firefighting system is dated 05DEC2017. 74 SOLAS (14)II-2/14.2.2.1 ref MSC.1/Circ.1432

Airlock doors should be self-closing. Interior and exterior airlock doors were not self-closing. 74 SOLAS (14)VII/13.1 93 IGC 3.6.2

Emergency shutdown valves in liquid piping should fully close in all service conditions within 30 s of actuation. Upon activation of the ESD, the port side manifold liquid line valves closed in 42 s. 74 SOLAS (14)VII/13.1 93 IGC 5.6.4

The company should establish procedures to ensure the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. In meeting these requirements, the company should ensure that: (10.2.1) inspections are held at appropriate intervals; (10.2.3) appropriate corrective action is taken. The vessel is required to complete annual servicing of the fixed dry powder fire-fighting system, and appropriate periodic calibration and testing of the fixed gas detection systems. Vessels previous fixed dry powder fire-fighting system annual servicing certificate was dated 05DEC2017, and was not carried out during previous ports of call in Ain Sukhna Egypt; Rabigh, Saudi Arabia; Antwerp, Belgium; Stade, Germany; or Vlissingen, Netherlands. During examination, it was discovered that servicing was scheduled for Houston, TX. 1.5 months after annual certification expiry. In addition, crew was unable to find a valid copy of calibration of fixed gas detection system, calibration certificate (labeled LGE TA-001) was blank, and the Master could only provide original shipyard testing certificate (STX Onboard Test Procedure, pg 45) dated 26OCT2016. The crew then was unable to demonstrate satisfactory on-board calibration of the fixed gas detection system resulting in erroneous LEL readings, which provides objective evidence that the Company did not provide satisfactory training to the crew. 74 SOLAS(14)IX/3.1 ISM Part A/10.2
