Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Vessel reported overheating to the Port Azimuth Thruster due to a failed coolant pump while transiting the Lower Granite Locks. Vessel is to provide a technicians report and evidence of permanent repairs to the satisfaction of Marine Inspector.

A door in a main vertical zone bulkhead must be of the self-closing type capable of closing against a 3.5 degree list of the vessel. The starboard aft salon fire door on the 4th deck and the aft atrium fire doors on the second deck were incapable of self-closing with the vessel on an even keel.

A door in a fire control boundary must be capable of operation from either side by one person. The starboard aft atrium fire door on the 3rd deck was incapable of opening from the atrium side.

Nontank vessels may not continue to operate on the navigable waters of the United States if the period of the vessel response plan [VRP] authorization has expired. Vessel's VRP expired in August 2023.

Every vessel must have a detailed inspection of the radio installation by an FCC-licensed technician once every five years. Vessel's Safety Radio Telephony Certificate has expired. 47 CFR 80.903

07105- Construction and arrangement of the a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of inspection giving consideration to provision for protection against fire. Vessel was found with 02 fire doors leading into the main dining area with a latch key missing proper fastners to ensure operation and one key on the door adjacent to the dining hall missing. Repair to the satisfaction of the attending marine inspector. 46 CFR 116.100(a)

7105- Construction and arrangement of the a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of inspection giving consideration to provision for protection against fire. 2nd deck door to the skylounge was found with a seized latch and unable to close. Repair to the satisfaction of the attending marine inspector. 46 CFR 116.100(a)

07120- Means of Escape - Each level of a space containing a balcony must have two independent means of escape that meet the requirements of § 116.500 of this part. Vessel egress dividers on the stbd fwd 2nd deck were found with bolts permanently affixing them to vessel and inhibiting their ability to be removed for emergency egress. Vessel shall remove the bolts and the egress dividers to their original design. 46 CFR 116.439

07103 - Division - Penetrations - Windows in doors in fire control boundaries must comply with the requirements of paragraphs (a) through (e) of this section. Glazing beads or angles of steel or equivalent material must be installed to hold glass in place in windows or air ports in a fire control boundary in event of a fire. Vessel had 05 window retention brackets missing on 2nd deck windows. Vessel is required to repair to the satisfaction of the attending marine inspector. 46 CFR 116.433

07115- Fire Dampers - Suitable means, such as a manual damper, automatic damper, or vent cover, must be provided in an accessible location outside the space served by the ventilation duct for shutting off the passage of air through the ventilation duct in the event of fire. FWD STBD laundry room had fire damper with missing handle to manually close damper. 46 CFR 116.610 (d)

09209 - Electrical- Electrical equipment on a vessel must be installed and maintained to: minimize accidental personnel contact with energized parts. Vessel had an open electrical junction box on the port overhead of the main engine room. Vessel is required to repair the junction box to the satisfaction of the attending marine inspector. 46 CFR 120.200

11117 - Lifebuoys - Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. Stern lifebuoy on the port side was found in unserviceable condition: tears in the body of the lifebuoy and deteriorated buoy line. Vessel is required to replace the lifebuoy and provide the attending marine inspector documentation. 46 CFR 115.808(b)

99101 - Other - All escape hatches and other emergency exits used as means of escape must be marked on both sides in clearly legible letters at least 50 millimeters (2 inches) high. Escape in the dining room markings were faded. Vessel is required the markings to the satisfaction of the attending marine inspector. 46 CFR 122.206

07199 - Other (Fire Safety) Electrical equipment on a vessel must be installed to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment. Vessel had an extension cord run as a permant installation through the overhead. Vessel removed extension cord from space. 46 CFR 120.200

07107 - Fire Patrol - Vessels described by 46 CFR 114.110(f) must submit plans to the cognizant OCMI, in accordance with 46 CFR 115.700, for the installation and use of monitoring device(s) to ensure the wakefulness of the watchmen required in paragraph (a) of this section. Vessel submitted plan states Each Lone Worker radio is programmed to emit 30 second audible warning alarm if the Security Watchstander does not manually key microphone every 15mins and a continual audible emergency alarm will sound on every shipboard handheld radio until the watchstander radio is manually keyed. Lone worker radio does not activate a continual audible emergency alarm on every shipboard handheld radio. Vessel is required to bring radios in compliance with approved watchman plan to the satisfaction of the attending marine inspector. 46 CFR 122.410

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. Vessel did not have the proper donning instructions for the lifejackets present onboard. Replace or install the lifejacket donning instructions in each crew/passenger stateroom to type present to the satisfaction of attending marine inspector. 46 CFR 122.516

EACH SURVIAL CRAFT AND LAUNCHING APPLIANCE ON A VESSEL MUST BE INSPECTED MONTHY USING THE MANUFACTURERS INSTRUCTIONS. VESSEL HAS NOT BEEN CONDUCTING MONTHY INSPECTIONS ON VESSELS SURVIVAL CRAFTS. VESSEL MUST PROVIDE DOCUMENTATION TO THE COAST GUARD THAT SURVIAL CRAFT MONTHLY INSPECTIONS ARE BEING CONDUCTED AND LOGGED.

The exhaust piping must be properly supported by noncombustible hangers or blocks. The support bracket on the starboard generator was found to be broken free from the block for the exhaust pipe. 46 CFR 119.430(b)

A vessel must be operated in such a manner as to afford adequate precaution against hazards that might endanger the vessel and the persons being transported. The coolant hose from the head tank for the centerline generator was found to be leaking.

Vessel must make permanant repairs to the 3 fractures identified in the superstructure that have been drill stopped and sealed as a temporary repair.

Penetrations and openings in watertight bulkheads must have means to make them watertight. Two aft bulkhead penetrations in machinery space are not watertight. Remove plastic hose and make cable penetrations watertight. Plastic hose in question is a chaffing item installed by Caterpillar for the Tier IV engine system. To be completed by 05APR, in order to permit time for company to consult with manufacturer.

Piping, including valves, pipe fittings and flanges, conveying vapor, gas, or liquid, the temperature of which exceeds 150F, must be insulated where necessary to prevent injuries. Each SSDG in engine room has unprotected areas on the turbo charger, potentially exposing crew members to injury. Install proper insulation/lagging on exposed areas within two weeks.

Penetrations and openings in watertight bulkheads must have means to make them watertight. Over 50% of all watertight penetrations are improperly installed IAW Coast Guard Approval 164.138/33/0 and exhibit unauthorized bundled cabling. Make watertight and correct improperly installed watertight penetrations.

Fire dampers must fitted with an indicator showing whether the damper is open or closed, and be marked with red letters of at least 0.5 inches in height stating, “VENTILATION FIRE DAMPER”. All fire dampers with the exception of 03 dampers protecting the galley space, are not properly marked or fitted with indicators.

A fire damper must close against the draft in the duct and be accessible for periodic inspection by means of a hinged or bolted plate in the duct and surrounding bulkhead or deck, if fitted. All internal fire dampers on board are not accessible for visual inspection.

A vessel with the accumulative oil capacity of 250 Barrels (10,500 Gallons) or more must have transfer procedures as required by 33 CFR 155.720 through 155.760; Persons in Charge as per 33 CFR 155.700 who are designated in writing as per 33 CFR 155.715, who meet the requirements of 33 CFR 155.710. The vessel must also meet all other applicable operational and administration items as identified in 33 CFR 155 and 33 CFR 156.115 through 156.170. Provide transfer procedures, line diagrams, PIC designations and proof of training and prove compliance with all applicable sections of 33 CFR 155 and 33 CFR 156 to the attending Marine Inspector prior to carrying out fuel transfer operations.
