Doors in "A" Class bulkheads shall meet the following requirements: Doors shall have a latch with a minimum throw of 3/4 inch which can be operated from either side of the door. MI observed door in the "A" class boundary bulkhead between the machinery space and workshop to be missing the handle and latching mechanism causing the door to remain open and have an open penetration. Install proper latching mechanism and door handle to meet the "A" class boundary requirement of the bulkhead. Additionally, maintain accepted mitigation procedures for entry and working within the workshop until proper repair/installation of door latch and mechanism is installed to the satisfaction of attending Marine Inspector. 46CFR32.56-35(a) 46CFR72.05(b)(3)

For the purpose of Rule 30 (72 COLREGS),a vessel at anchor includes a barge made fast to a buoy. Such barge may be lighted as a vessel at anchor or may be lighted on the corners in accordance with 33 CFR 83.30h. Vessel was not using an anchor light when moored to buoy. 33 CFR 82.5

Each berth must have a light. Vessel does not have installed lights above the beds. Portable ones have gone missing. 46 CFR 92.20-45.

Every portable fire extinguisher must be supplied with a suitable bracket. Portable extinguisher was not in the bracket required by manufacturer. 46 CFR 162.028-3(c)

Sub-Chapter S requires every vessel to have a stability letter. The vessel has been found to be operating outside the parameters of their stability letter. Vessel shall remove ballast or have a new stability letter issued. 46CFR31.10-3 46CFR170.120

During an internal structure exam required by 46CFR31.10, two failed welds were discovered; First one is IWO the side shell seam weld, in Cargo Hold #1 STBD side. 5 frames FWD of the access hatch and 7 sidelongs from the bottom. The second weld is in the same location but the actual sidelong itself had been split at the butt weld as well. 46CFR31.10-21(c)

Where the general emergency alarm signal cannot be heard over the background noise, there must be a red flashing light or rotating beacon, in addition to the general emergency alarm signal. Flashing light in generator space did not operate.

Each general emergency alarm signal must be marked "GENERAL ALARM—WHEN EMERGENCY ALARM SIGNAL RINGS GO TO YOUR STATION" in red letters at least 1/2 inch high.

Lock out/tag out newly installed stb aft mooring winch until Coast Guard inspection complete.

Barge has a sea chest and sea valve in #2 void. After installation of a snow removal system, vessel should have meet the requirements of 46 CFR 56.50-55, but did not. Provide approved plans and followed by installation of a bilge pump prior to operation of the snow removal system.

Due to incorrect load line certificate use draft marks on vessel stability letter for both manned and unmanned conditions.

Make lower pressure vessel relief valve reset within 10% of set limit.

As a result of grounding on the night of 11/15/18, vessel was verbally directed to standby, barring exigent circumstances, for a damage survey by a USCG Marine Inspector and to provide results of a dive survey of the hull.
