All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in a normal manner.
PSCO observed exposed electrical wiring within the 440V junction box for a hydraulic pump unit and the associated breaker was in the "ON" position. PSCO observed missing indicator switch for a 440V connection in the Gyro Room.
Additionally, there were missing electrical switches in the Gyro Room for 220V connections as well as deck electrical switches for 220V connections, light fixtures, and deck house light fixtures. Vessel did not properly implement tag-out procedures to prevent electrical shock. 16A/C

PSCO observed flying bridge speaker unwired in damaged junction box as well as aft freefall lifeboat speaker cables left hanging and unwired. PSCO also observed missing communications speaker for emergency towing, mooring, and general communications.
