All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in a normal manner. PSCO observed exposed electrical wiring within the 440V junction box for a hydraulic pump unit and the associated breaker was in the "ON" position. PSCO observed missing indicator switch for a 440V connection in the Gyro Room. Additionally, there were missing electrical switches in the Gyro Room for 220V connections as well as deck electrical switches for 220V connections, light fixtures, and deck house light fixtures. Vessel did not properly implement tag-out procedures to prevent electrical shock. 16A/C

To prevent the ignition of combustible materials or flammable liquids, means shall be provided to limit the accumulation of flammable vapours. PSCO observed Gasoline cans and sealing foam aerosol cannisters were found in #1 Mast House without fixed fire suppression. 74SOLAS20 II-2/4.1.2 10c

No electrical equipment shall be installed in any space where flammable mixtures are liable to collect, e.g. in compartments assigned principally to accumulator batteries, in paint lockers, acetylene stores or similar spaces, unless the Administration is satisfied that such equipment is appropriately certified for safe usage in the dusts, vapours or gases likely to be encountered. PSCO observed cracked explosion-proof light fixture in the paint locker. 74SOLAS20 II-1/45.10.4 (16a/c)

Except as permitted by the Administration in exceptional circumstances, all metal sheaths and armour of cables shall be electrically continuous and shall be earthed. PSCO observed multiple wasted bonding straps on deck for ventilation shrouds, crane E-stops, and 220V junction boxes. 74SOLAS20 II-1/45.5.1 (16 a/c)

An emergency means comprised of either fixed or portable equipment or both shall be provided for two way communications between emergency control stations, muster and embarkation stations and strategic positions onboard. PSCO observed flying bridge speaker unwired in damaged junction box as well as aft freefall lifeboat speaker cables left hanging and unwired. PSCO also observed missing communications speaker for emergency towing, mooring, and general communications. 74SOLA20 III/6.4.1 (16 a/c)

The administration shall be satisfied that adequate arrangements are in place to ensure that the performance of the equipment required by this chapter is maintained. PSCO observed the float-free VDR hydrostatic release unit and VDR capsule battery to be expired. 74SOLAS20 V/16.1 (16a/c)

Maintenance, testing and inspections shall be carried out based on the guidelines developed by the Organization and in a manner having due regard to ensuring the reliability of fire-fighting systems and appliances. PSCO observed forward starboard fire hydrant gauge wasted and non-operational 74SOLAS20 II-2/14.2.2.1 (50a/c)

The master, owner, operator, agent, or person in charge of a vessel equipped with ballast tanks that operates in the waters of the United States must employ one of the following ballast water management methods. Vessel did not use their ballast water treatment system and only did a mid-ocean exchange. 33CFR151.2025(a) 40c

The safety-management system shall be maintained in accordance with the provisions of the ISM Code. PSCO observed that weekly checklist for fuel level was checked off as being full 05 days ago. PSCO observed that the weekly checklist was filled out as being completed for the rest of the month.
