The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull, protection against fire, means of escape in case of a sudden unexpected casualty, guards and rails in hazardous places, ventilation of enclosed spaces, and necessary facilities for passengers and crew. Vessel reported excess water in bilge. Upon further examination a pin hole leak was found in the vicinity of the stern tube. Vessel is restricted from moving from its current position at the Central Bay WETA facility until temporary repairs are affected to the satisfaction of the attending Marine Inspector. 46 CFR 116.100(a)

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed, and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Vessel lost both main engines while transiting to SF Ferry building and port side allided with Pier 14. Following recovery of both main engines, vessel moored and offloaded passengers. Vessel authorized one time transit without passengers to Alameda WETA facility. Conduct assessment of any damage sustained, provide root cause of engine failures, provide OCMI with damage assessment and rectification of engine failures. 46 CFR 119.200

A-Class bulkheads must be composed of steel or equivalent material, suitably stiffened and made intact with the main structure of the vessel...In addition, they must be so insulated with approved structural insulation, bulkhead panels, or deck covering. Identified cut open structural fire protection on starboard side of port engine room. 46 CFR 116.415

Penetrations and openings in watertight bulkheads must have means to make them watertight. Restore watertight integrity of cable transit in starboard forward bulkhead in the fuel tank compartment. 46 CFR 171.114

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: Charts of large enough scale to make safe navigation possible; U.S. Coast Pilot or similar publication; Coast Guard Light List; tide tables; and current tables. Extracts from the publications listed above for the areas to be transited may be provided instead of the complete publication. Vessel did not have required publications. 46 CFR 121.420

Penetrations in A-Class fire control boundaries for electrical cables, pipes, trunks, ducts, etc. must be constructed to prevent the passage of flame and smoke for one hour. In addition, the penetration must be designed or insulated so that it will withstand the same temperature rise limits as the boundary penetrated. There are 04 cable penetrations that are not properly sealed. 46 CFR 116.415(a)(2)(ii)

Master/Operator reported that they experienced an unplanned shutdown of MPDE #1. Master/Operator shall provide USCG with a detailed report of findings and root cause analyses related to unplanned engine shut down.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of machinery, fuel, and piping systems, including operational test of the means provided for pumping bilges. Connection between hard piping and bilge pump in port side lazarette is leaking and does not provide proper suction of water from the bilge. 46 CFR 115.804(h)

The fire control plan must show structural fire protection boundaries. Current plan outlines A class boundaries but does not specify A-0, A-15, A-30, or A-60. Provide new fire control plan with specific A class boundary type.
