The last 1.0 meter of vapor piping upstream of the vessel vapor connection and each end of the vapor hose must be painted in the sequence of red/yellow/red and labeled with the word vapor. PSCO observed vapor manifold connections missing .01 meter red band at end connection on all 4 vapor connection points.

Each vessel vapor connection flange must have a permanent stud projecting outward that has a 12.7 mm diameter and is at least 25.4 mm long. It must be at the top of the flange face. midway between boltholes, and in the line with the bolthole pattern. Vessel's four VCS lines at manifold did not have stud at end of fitting.

In tankers, electrical equipment, cables and wiring shall not be installed in hazardous locations unless it conforms with standards not inferior to those acceptable to the organization. PSCO observed disconnected bonding straps in various locations out on deck (Zone 1) to include bunkering line.

Fire-Fighters outfits shall complay with Fire Saftey Systems Code. Vessel personal equipment shall consist of the following protective clothing of material to protect the skin from heat radiating from fire and from burns and scalding by steam. Vessels fire fighting outfits do not protect ears and the skin on the back of the neck. 74SOLA14/II-2.10.1.1
