The Coast Guard recorded this vessel and the following in the same

No extensive repairs to the hull or machinery which affect the safety of a vessel shall be made without the knowledge of the Officer in Charge, Marine Inspection. After off load at Ergon Dock MM456 TNR, contact local OCMI off load completed and for possible PTP for permanent repair destination. No further cargo operations till permanent repairs are completed to the satisfaction of the CG Marine Inspector.

12’ Rake / Tank Barge / Load of Styrene / 4 wing tanks
- commencing 18’ of the starboard number four forward bulkhead and extending aft 7’, the side is set in 0 - 1.75” for height with internals distorted to conform.
- the aft 8’ of starboard number four side is set in 0 - 1.75” over the upper 4’
- commencing 9’ 6” aft of the port number one forward bulkhead and extending aft 4’, the upper 3’ of side, deck and gunwale is set in & up 0 - 2” to varying degrees
- commencing 17’ forward of the port number one aft bulkhead and extending forward 8’, the upper 6’ of side is set in 0 - 2” to varying degrees
Vessel may offload in at MM 175.1 on the Ohio River in MSU Huntington's AOR then transit to MM 237 on the LMR for permanent repairs.
