The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. Vessel did not have emergency instructions on board. 46 CFR 185.510(a)

Fuel filters fitted with bowls of other than steel construction must be approved by the Commandant and be protected from mechanical damage. Port and starboard fuel filters were intended to be fitted with bowls meant to be highly resistant to shattering. Both fuel filters were missing bowls. 46 CFR 182.458(b)(2)

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. Vessel's first-aid kit missing aspirin and eye wash. 46 CFR 184.710

A vessel is inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by the standards in this subchapter may be inspected in accordance with standards acceptable to the OCMI as good marine practice. Identified several cotter pins installed on standing rigging at deck level that were installed without bending the pins back to prevent passenger or crew injury. Provide evidence of all deck level cotter pins installed in an acceptable condition that would prevent passenger or crew injury. 46 CFR 176.800(a)

The preventative maintenance plan (PMP) should address the following at a minimum: the maintenance schedule of the rigging and associated equipment to include detailed protocol for examinations and inventory of rigging components including age of component as well as size and replacement schedule. The operator has not established a PMP for this vessel that includes the age of components or replacement schedules and detailed protocol for examinations of rigging to be conducted by the crew. The operator must provide a PMP for this vessel. The PMP shall include at a minimum: the maintenance schedule of the rigging and associated equipment to include detailed protocol for examinations, inventory of rigging components including age of component as well as size, and replacement schedule (life cycle). A third party rigging survey does not substitute for the vessel owner/operator from implementing a PMP or conducting/documenting preventative maintenance. 46 CFR 176.840

A vessel must be fitted with FCC type accepted general marine radar system for surface navigation with a radar screen mounted at the primary operating station if the vessel carries more than 49 passengers operating on a limited coastwise route. A radar is not installed on the vessel and the vessel operates on a limited coastwise route with more than 49 passengers permitted. 46 CFR 184.404

Piping used in a vital system must be composed of ferrous materials. The bilge piping on the starboard manifold for the mid section piping has a temporary patch made of PVC. Replace as original. 46CFR182.710(c)(1)

An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. Both port and starboard engines were missing guards over the pulleys and belts. Place guards on engines as needed to prevent injury. 46CFR177.960

Each ring type life buoy must be marked with type II retro-reflective material approved in accordance with 46CFR 164.018. The additional life buoy on the aft, port side of the vessel is missing retro reflective tape. Add retro-reflective tape. 46CFR185.604(i)

A vessel must comply with the applicable requirements for any radio installation, including the requirements for a station license and installation certificates to be issued by the Federal Communications Communications, as set forth in 47 CFR part 80. Vessel was missing a copy of the issued radio telephony certificate for VHF. 46CFR184.502

Prove pre-engineered fixed gas fire extinguishing system automatic shut down and override system is operational. 46CFR181.420

The vessel did not have an appoved Stability Letter. The Dead weight survey was conducted 11 DEC 2017 and test results have been sent to MSC.
