OMO shall be prepared to test the steering systems of the vessel and make them available for inspection to the extent necessary to determine that they are in suitable condition and fit for the service intended.
During underway steering test, the vessel's master lost maneuverability. MI's also observed water starboard rudder post in the lazarette in a slow-acting steering ram. Make satisfactory repairs to the attending Marine Inspector.

Resolution: Rudder was repacked,and the steering system was bled of air. Conducted underway maneuverability test, sat.
The vessel was involved in a marine casualty and sustained extensive damage to the hull, resulting in flooding of the engine room and steering compartment. The vessel's structural integrity has been compromised, and the extend of damage raises significant concerns regarding watertight integrity, stability, and safe operation. A comprehensive survey must be conducted and a repair plan be submitted and accepted by the Coast Guard for review. The vessel is restricted from operations until the requirement is met and verified by a Marine Inspector. Submit a detailed hull damage survey and proposed repair plan. Repairs must be completed to the satisfaction of the OCMI and verified prior to resuming operations.

Resolution: While onboard, the team verified the condition of the genset fuel line fittings, bilge pump PTO, and verified servicing had been completed for the pre-engineered fire extinguishing system. All identified issues have been satisfactorily repaired.
A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford adequate precaution against hazards that might endanger the vessel and the persons being transported. Coast Guard received a report of water ingress in the engine room through the stern tube. After further investigation it was determined that there is a crack beneath the waterline with a steady stream of water ingress.
Vessel must submit a repair plan and complete repairs satisfactory to the attending Marine Inspector prior to vessel movement.
