Expired agreement letter with drug Third Party Administrator (TPA). Need to provide current drug consortium TPA list of all vessel crew members that are subject to random drug testing.

Vessel EPIRBS must be tested monthly and logged. No log entries for EPIRB tests at time of inspection. Corrected on the spot.

Vessels not required to have a power driven fire pump must have at least three (03) 2.5 gallon buckets with lanyards. No fire buckets present at time of inspection.

All EPIRBs must be tested and logged on a monthly basis. Logged EPIRB tests not available for inspection. Corrected on the spot.

Vessels on a limited coastwise route are required a minimum of 12 distress signals. On 10 of the required 12 were on board. Provide 2 additional approved distress signals. 46 CFR 180.68

Vessels are required to carry an approved first aid kit or the contains thereof. The inhalants and aspirin was expired. Provide a current first-aid kit or equivalent. 46 CFR 184.710

Each vessel must be equipped with at least 2 portable battery lights. Only 1 light was provided. Provide an additional portable battery light. 46 CFR 183.430

Operation of a vessel on a route of lesser severity than those specifically described or designated on the Certificate of Inspection is permitted unless expressly prohibited on the Certificate of Inspection. The general order of severity of routes is: oceans, coastwise, limited coastwise, Great Lakes, lakes, bays, and sounds, and rivers. The cognizant OCMI may prohibit a vessel from operating on a route of lesser severity than the primary route a vessel is authorized to operate on if local conditions necessitate such a restriction. The vessel has been operating on the eastern side of Hawaii, which constitutes exposed waters per Sector Honolulu WI 06. The vessel's October 25, 2017, Stability Letter approved the vessel to operate on Partially Protected Waters. The vessel is only allowed to operate on the Western coast between a line extending due west from Upolu Point and a line extending due west from Kauna Point. The vessel needs to provide a new stability letter that states it is allowed to operate on Exposed Waters which is the eastern side of Hawaii.

All life floats on vessel need to be in good working order. Repai and/or replace life float.

Replace hydrostatic release on EPIRB.

Vessel company needs to provide all required documentation for its drug and alcohol testing program.

Vessel needs to replace 05 flares with an approved type.

Vessel needs to post an emergency instruction list onboard for passengers to view.

Vessel needs to have onboard all required charts and publications.
