Install and operate a ballast water management system that has been approved by the Coast Guard under 46CFR162. PSCO observed ballast water management system inoperable due to faulty TRO sensor

If the intalled BWMS required by this subpart stops operating properly during a voyage, or the vessel's BWM method is unexpectedly unavailable, the person directing the movement of the vessel must ensure that the problem is reported to the nearest COTP or District Commander as soon as possible. BWTS stopped operating properly on 05MAY2026 at 1000 local time. Inoperable equipement was not reported until 06MAY2026 at 0630 local time.

To discharge ballast water into waters of the United States, the master, owner, operator, agent, or person in charge of a vessel is subject to 151.2025 of this subpart must either ensure that the ballast water meets the ballast water discharge standard as defined in 151.2030(a), use an AMS as described in 151.2025(a)(3) or ballast exclusively with water from a U.S. public water system, as described in 151.2025(a)(2), according to the schedule in paragraph (b) of the section. PSCO observed discharge or approximately 13,891m^3 of untreated ballast water in Galveston, TX without authroization.

Ignition sources shall be restricted. PSCO identified multiple areas of oil soaked lagging on heavy fuel oil pumping system and fuel oil purging system.

The machinery and associated fitting shall be adequate for the service in which they were intended. The packing glands on all general service and cooling water pumps were leaking water.

The normal equipment of every lifeboat shall consist of a food ration of not less than 10,000kJ for each person on board. These rations shall be kept in airtight packaging. 01 ration in Port Lifeboat found non-airtight.

To assist in casualty investigation, cargo ships, when engaged on international voyages shall be fitted with a simplefied voyage data recorder as follows... Vessel's back up battery expired December, 2020.

The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevent rules and regulations and any additional requirements which may be established by the company. Inspection in Feb 2020 indicated that the VDR battery backup was to expire in Dec 2020. Vessel failed to utilize procedures to replace batteries, to maintain the vessel in conformity.

The condition of the ship and its equipment shll be maintained to conform with the provisions of the present regulations to ensure the ship in all respects will remain fit to proceed to sea without danger to personons on board. When PSCO opened ships fixed gas detection cabinet he was able to detect H2S present in that space. It was observed that the sample line for cargo vapor line was left open and leaked into cabinet.
