The various tasks should be defined and assigned to qualified personnel. Vessel did not treat ballast water on board per U.S. regulations. The vessels ballast water management plan (BWMP) has clear guidance on local regulations, however, the company provided "Check list for vessels calling US/Canadian Ports, form OP-17" has conflicting information that does not inform the master of BWM Practices. 74SOLAS(14) IX/3.1 ISM Code Part A/7 50ac

Certified safe type equipment may be fitted in gas dangerous spaces and zones in accordance with the following provisions. Instrinsically safe electrical equipment and qiring may be fitted in all gas dangerous spaces and zones as defined in 1.3.17. Inside cargo compressor room, on the stbd side aft bulkhead, the fresh water and sea water cooling pump start switches were found to have no intriscially safe seals where the wires entered the terminal box. The circuits and wiring could be seen from below the box through an open hole approximately 30-40 mm wide. Crew temporarily sealed the fitting with putty. 74SOLAS(14) VII/13.1 IGC 10.2.1 40ac
