At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. During operational testing, the rubber seal on the port intermediary rod between the engine room and azipod room was smoking, creating a fire hazard and rendering the vessel's primary steering system unfit for the service intended. Provide root cause analysis of propulsion system issue,and conduct satisfactory sea trials to MI satisfaction. 46 CFR 176.830(a)

During operational sea trials, MI witnessed (EVC)system experienced intermittent communication faults to PORT POD steering unit. Provide root cause analysis of propulsion system issue,and conduct satisfactory sea trials to MI satisfaction, prior to carrying passengers-for-hire.

Whenever damage or deterioration to hull plating or structural members that may affect the seaworthiness of a vessel is discovered or suspected, the cognizant OCMI may conduct an ISE in any affected space and may require the vessel to be drydocked or taken out of service to assess the extent of the damage, and to effect permanent repairs. MIs received report that vessel made contact with a pier and sustained damage along outward portion of the starboard hull. Operator effected repairs without MI oversight or assessment of damage. Vessel is restricted from carrying passengers until MI attends to assess the condition of the repair. 46 CFR 176.600

Emergency fuel shutoff sign on either side of aft main cabin is directly above manual release for pre-engineered fire system. Fuel shutoff pull is beneath fire system pull and has the potential to inadvertently pull the wrong system in an emergency. Arrange pull stations with proper safeguards to ensure incorrect line is not triggered.
