Multiple instances were evident that the vessels Lock out Tag Out (LOTO) system does not ensure that tags were removed by the person who placed the tag. Additionally, LOTO was being used to place out of service components and equipment for purposes other than repair work. Audit the existing LOTO/installed tags and correct as necessary.

Per 14.12.5 of the 1989 MODU Code, each lifeboat should, as far as practicable, be launched with its assigned crew aboard and manuevered in the water at least once every 3 months. Per paragraph 11 of MSC Circular 1486 Annex, supplemental training cannot take the place of launching when favorable weather conditions are provided during the 3 month period. Lifeboats were not launched once every 3 months as required. Provide evidence of launching all lifeboats.

Extended to 7Mar18. RSM. Per 9.1.3 of the 1989 MODU Code, the minimum fire integrity of bulkheads and decks should be as prescribed in tables 9-1 and 9-2. Bulkheads should prevent the passage of flame/smoke and per 3.6.1 shall ensure watertight integrity of enclosed compartments. Multiple marine cable transits and doors were incorrectly installed using unapproved materials and/or lacked original packing that reduced their fire integrity. Survey all marine cable transits, doors, and other components to ensure fire integrity is provided.
