Electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. The vessel was returned from charter in an unsafe condition. Known safety concerns include a missing generator and shock hazards from distribution panels. Vessel to be made available for a Coast Guard in-service inspection, or, a TPO external vessel survey prior to returning to commercial towing service. IAW 46 CFR 144.135, any repairs that are not "Replacement In Kind" must have plans verified as compliant with design standards as identified in 46 CFR 144.145 prior to the new installation is performed. (Replacement in kind means replacement of equipment or components that have the same technical specifications as the original item and provide the same service. If the replacement item upgrades the system in any way, the change is not a replacement in kind - 46 CFR 136.110).

The owner or managing operator of the towing vessel must make a request for an amended COI to the cognizant OCMI any time there is a change in the character of the vessel, its operation, or similar factors specified in its current COI. AS of this date this office has not received a request for an amended COI when certain factors, as stated on the COI, are no longer current. Specifically, the vessel is no longer operated by Mardi Gras Marine and the vessel is no longer covered by TSMS Certificate ITOW-MGM-20240306. The vessel must satisfy all requirements to receive an amended COI, which may include an OCMI inspection, prior to conducting commercial towing operations.

Ensure the type 1 and type 5 PFD's are segregated and all enclosed type 1 storage locations need to be labeled.

Remove or replace the bow life buoy. It is in excess and has no approval number on it.

Ensure all engine exhaust and turbos that exceed 425* temperature are properly insulated.

Install a second G/A light in the upper engine room.

Ensure the vessel has an operating white forward mast head light and stern light. The vessel currently has only one white 360* mast head light which is only acceptable on a vessel less than 40’.

Ensure the two amber towing lights use approved bulbs.

Properly secure the port galley door (the door hinge is loose and will not close/open without excessive force). .

Ensure the new crew training which is required by the safety and health manual is conducted within the 5 day requirement.

Ensure the vessel has non-flammable vent closures installed. Vessel currently uses canvas and with the protruding stack exhaust pipis, canvas will only catch on fire if installed while the engines/gens are running or the exhaust pipes are still hot.

Vessel did not conduct an annual COI exam prior to 90 days past their anniversary date. Vessel must conduct a COI exam and receive a valid COI prior to commercial use.

Prior to sailing, Vessel must provide evidence of completed fire drill, crew member orientation, and health and safety plan training.

Prior to sailing, Vessel must have 02 serviceable lifebuoys with lights. In addition 01 lifebuoy must have a line attached.

Prior to sailing, Vessel must have Professional Engineer's letter for fire panel.

Prior to sailing, Vessel must have up to date charts and navigation publications.

Prior to sailing, Vessel must ensure all PFD's onboard are properly label with vessel name and are equipped with a whistle and a light.

Prior to sailing, Vessel fuel shutoff must be labeled and old unused shutoffs must be removed.

Prior to sailing, Vessel must ensure means are provided to close all openings to machinery space that could permit air influx in case of fire.

Prior to sailing, Vessel must have an operating accurate compass or swing meter in wheelhouse.

Prior to sailing, Vessel must have working general alarm.

Vessel must ensure breakers in wheelhouse panel are properly labeled.

Vessel must ensure TVR has entries for inspection and testing of emergency lighting and storage batteries every 3 months.

Vessel must ensure all wiring penetrations in void spaces under wheelhouse are protected from chafing.

Vessel must remove or replace chaffed wiring for light in starboard side wheelhouse void.

Vessel must remove Styrofoam insulation in wheelhouse void.

Vessel must ensure lights in bunk rooms are guarded to prevent accidental crewmember contact with bulbs.

Vessel must ensure all light fixtures have appropriate bulbs installed to eliminate shock hazard.

Vessel must ensure that all bunkroom mattresses have appropriate fire ratings.

Vessel must remove all unused fire equipment mounting brackets and signage.

Crop and renew corrosion on the 2nd deck catwalks in the vicinity of the port and starboard bunkroom doors.

Vessel must ensure all switches and outlets in galley are provided with appropriate covers.

Vessel must ensure that breaker panel in 1st deck bunkroom has a door and is properly labeled.

Provide guarding for exhaust fan in main deck head.

Provide a cover on the heater mounting enclosure with exposed wiring in the main deck head.

Replace the missing piece of handrail on the main deck port side in the vicinity of the galley door.
