Each vessel must have adequate emergency lighting fitted along the line of escape to the main deck from all passenger and crew accommodation spaces located below the main deck. MIs observed emergency lighting flickering on and off in the STBD engine room and interior passenger seating area. Repair to the satisfaction of attending MI.46 CFR 183.432

Resolution: Replaced breaker. emergency lighting properly working. Cleared to the satisfaction of the Coast Guard.

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull. MIs observed quarter-sized hole in STBD bow void space. Submit repair plan, company statement attesting to cause of damage, and action plan to avoid future reoccurring damage to the satisfaction of the attending MI. 46 CFR 177.100

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Vessel MDE mounting bolts were loose. Repair to the satisfaction of attending MI. 46 CFR 182.220

Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Vsl Suffered small fire on portside generator starter. Repair affected machinery to factory and to the satisfaction of an attending Marine Inspector. Prior to getting the vessel back into operation contact Sector New Orleans dispatch for an inspection of the repairs. 46CFR182.100

The RTA2 is currently restricted form underway operations until the root cause analysis of the PMDE starter failure has been accomplished and corrective action taken. Prior to getting the vessel back in operation contact the SECTOR New Orleans Coast Guard for an inspection of the vessel.
