Electrical installations should be such as to minimize the risk of fire and explosion from flammable products. Numerous cable glands within the cargo machinery room were found missing plugs/stoppers which allow gas to enter electrical boxes, as well as missing packing glands.

Mechanical ventilation inlets and outlets should be arranged to ensure sufficient air movement through the space to avoid the accumulation of flammable or toxic vapors and to ensure a safe working environment. The cargo machinery room was not suppling adequate air movement to prevent accumulation of flammable vapors. Coast Guard inspectors and vessel crew Gas meters were showing 19.1 oxygen and 8% LEL while inside cargo machinery room.
