M/V ROALD AMUNDSEN failed to adhere to the notice of arrival requirements set forth in 33CFR160.212. The NOA was not submitted in the required timeframe.

The vessel transited the water of the Prince William Sound without a VRP approved for operation in PWS.

Tanks,voids, and auxiliary spaces having little or no fire risk; Auxiliary spaces which do not contain machinery having a pressure lubrication system and where storage of combustibles is prohibited. Fire screen doors shall be self closing and capable of closing with an angle of inclination of up to 3.5 degrees opposing closure. Fire screen door MVZ 2 deck 8 2814 did not close and latch completely.

Interior locations in passenger ships carrying more than 36 passengers, fire hoses shall be connected to the hydrants at all times. Several fire hoses were not connected to hydrants throughout the ship.

Embarkation ladders shall be free from knots or other irregularities. Port Aft embarkation ladder shows signs of dry rot.

On transitional power, emergency power shall provide emergency lighting in alleyways, stairways, and exits giving access to muster and embarkation stations. Vessel was issued work list item during initial COC to verify proper operation of all emergency lighting throughout vessel on emergency/transitional power.
