Audible and visual alarms shall be provided to indicate oxygen content in excess of 5% by volume. The fixed oxygen analyzer is inoperable and cannot indicate content to provide alarm vessel is operating with a letter of dispensation and running the I.G.S. with portable oxygen meters.

No electrical equipment shall be installed in any space where flammable mixtures are liable to collect including those on board tankers, unless the Administration is satisfied that such equipment is appropriately certified for safe usage in the vapours likely to be encountered. Numerous electrical hazards found in the cargo pump room, to include loose wiring and connections on the following items: port bilge alarm, ODME flow sensor, light fixture on 2nd platform, light fixture and junction box on 1st platform, smoke detector below 1st platform.

Aftert any survey of the ship has been completed, no changes shall be made in the machinery, equipment, and other items covered by the survey, without the sanction of the Administration. PSCO observed I.G.S. with both blower inlet valve handles, and electrical indicators dismantled and in poor condition. Valve atmosphere at local indicator does not appear to be operating and has inoperable pressure gauge.
