The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull, protection against fire, means of escape in case of a sudden unexpected casualty, guards and rails in hazardous places, ventilation of enclosed spaces, and necessary facilities for passengers and crew. Vessel has a port engine fire. Vessel is authorized to have the vessel towed to it home berth once released by Orange County Sheriff. USCG Marine Inspector must attend the vessel and inspect repairs prior to code 60 being lifted.

It is intended that all machinery be installed and maintained in such a manner as to afford adequate protection against fire, explosion, machinery failure and personnel injury. MI observed PORT engine J/W hose with excessive cracking at both ends behind the clamp. Replace hose and make available for inspection to the attending Marine Inspector.

The person in command of a documented vessel must have on board that vessel the original certificate of Documentation currently in effect for that vessel. Original COD was not onboard the vessel.

Prior to departing on a voyage, the passenger count must be communicated verbally or in writing, and available ashore at the vessel's normal berthing location or with a representative of the owner or managing operator of the vessel. Vessel does not have procedures for communicating the passenger count to shore personnel prior to departing on a voyage.

On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate a high water level. Forepeak visual alarm was inoperable.

A visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating. Visual indicator light for the automatic bilge pump was inoperable.

Each entrance to a space storing carbon dioxide cylinders, a space protected by carbon dioxide systems, or any space into which carbon dioxide might migrate must be conspicuously marked as follows: Spaces protected by carbon dioxide - "CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. WHEN ALARM OPERATES OR WINTERGREEN SCENT IS DETECTED, DO NOT ENTER UNTIL VENTIALTED. LOCK OUT SYSTEM WHEN SERVICING." Port and Starboard Engine Rooms did not have CO2 signage posted.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Electrical power strips found daisy chained in succession. Remove daisy chained power strips

Each terminal part of a conductor must be made within an enclosure and have either: (1) A pressure type connector on each conductor; (2) A solder lug on each conductor; (3) A splice made with a pressure type connector to a flexible lead or conductor; or (4) A splice that is soldered, brazed, or welded to a flexible lead or conductor. Several dead-end wires found throughout the vessel. Remove or properly terminate dead end wires.

A vessel must carry one floating waterlight, unless it is limited to daytime operation. Vessel’s ring buoy waterlight had incorrect battery. Provide appropriate waterlight battery and prove proper operation to attending USCG Marine Inspector.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including inspection and operation of all watertight closures in the hull, decks, and bulkheads including through hull fittings and sea valves. During underway inspection it was observed that the portside prop shaft stuffing box exhibited excessive leakage. Repair or replace shaft seal to prevent excessive leakage to the satisfaction of attending USCG Marine Inspector.

Portable fire extinguishers must be located so that they are clearly visible and readily accessible from the space being protected. The installation and location must be to the satisfaction of the Officer in Charge, Marine Inspection. Main accommodations space and below deck aft store room both fire extinguishes not properly mounted or clearly visible. Properly mount extinguishers in approved mounting bracket in a clearly visible location or provide signage.

The hailing port of the vessel must be marked on some clearly visible exterior part of the stern of the vessel. (d) The markings required by paragraphs (a), (b), and (c) of this section, which may be made by the use of any means and materials which result in durable markings, must be made in clearly legible letters of the Latin alphabet or Arabic or Roman numerals not less than four inches in height. Vessel does not have hailing port markings as required. Properly mark vessel with hailing port as required.

Overcurrent protection must be provided for each ungrounded conductor for the purpose of opening the electric circuit if the current reaches a value that causes an excessive or dangerous temperature in the conductor or conductor insulation.The battery switches and 2/0 cables leading to the inverters are not adequately protected by the 350A fuses, as required by 46 CFR 183.380(d).

The owner or managing operator shall submit drawings, sketches, or written specifications describing the details of any proposed alterations to the cognizant OCMI. Provide required information as listed in the USCG Marine Safety Center letter dated March 15, 2024. 46CFR176.700(b)

A vessel that is exposed to salt water more than 3 months in any 12 month period must undergo a drydock examination and an internal structural exam at least once every 2 years. Vessel is overdue for required drydock inspection as of 01MAR2023.

Where provisions are made for charging batteries, there must be natural or induced ventilation sufficient to dissipate the gases generated. Batteries for auxiliary systems in the accommodation does not have adequate ventilation. 46CFR183.350(a)

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces:(1) A space with a through-hull fitting below the deepest load waterline, such as a lazarette. High level alarm in the lazarette compartment does not operate as designed. 46CFR182.530

Each ring life buoy on a vessel must: (1) Be approved in accordance with § 160.050 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.150. Grab line on ring life buoy deteriorated beyond serviceability. 46CFR180.70(b)(1)

(1) Controls and valves for operation of fixed gas fire extinguishing system must be:(i) Located outside the space protected by the system. Natural ventilation closure controls are located in the space protected by the fire suppression system. 46CFR181.410(b)

On a vessel on which the number of passengers permitted on upper decks is limited by stability criteria, as indicated by the vessel's stability letter, the maximum number of passengers allowed on an upper deck must be indicated. Upper deck indicated 40 maximum passengers. COI limits max passengers to 34 personnel. 46CFR185.602(g)

Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10. 02 fire extinguishers were not secured with approved bracket system. 46CFR176.810(b)(1)

The person in command of a documented vessel must have on board that vessel the original Certificate of Documentation currently in effect for that vessel. Original COD was not on board the vessel. 46CFR67.313(a)

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician. Radiotelephony certificated is expired. Radiotelephony certificated is expired. 46CFR80.903

Electrical equipment on a vessel must be installed and maintained to: Provide services necessary for safety under normal and emergency conditions. Port side engine room does not have adequate lighting. 46CFR183.200(b)

Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. Jacket water hose on stbd engine is deformed. 46CFR182.100

Machinery and fuel tank spaces must be separated from accommodation spaces by boundaries that prevent the passage of vapors. Engine room doors not vapor tight. Repair or replace to the satisfaction of attending marine inspector. 46CFR177.405(c)

Footholds, handholds, ladders, and similar means provided to aid escape, must be suitable for use in emergency conditions, of rigid construction, and permanently fixed in position, unless they can be folded,yet brought into immediate service in an emergency. Emergency escape from aft compartment did not have a ladder ivo the escape hatch. 46CFR177.500(m)

A duct must be of rigid permanent construction, which does not allow any appreciable vapor flow except through normal openings. Duct system in engine room is non rigid. 46CFR182.465(d)

A vessel is inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Engine room has multiple areas of free communication to outside of vessel, compromising any vapor tightness in case of CO2 discharge. 46CFR176.800(a)

Deck rails must withstand a point load of 200 lbs in any direction. Aft rail and starboard bow rail fastenings loose and/or broken.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull. Port MDE shaft packing drips excessively (constant flow).

Your vessel must undergo an annual inspection within 3 months before or after each anniversary date of the COI. Vessel exceeded 3 month interval on 09FEB2021. Vessel is required to complete a satisfactory annual inspection prior to carrying passengers.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the [name withheld] of new Inverter System for Approval.

Gauges to indicate engine revolutions per minute (RPM), jacket water discharge temperature, and lubircating oil pressure must be provided for all propulsion engines installed in the vessel. Starboard side RPM indicator found to be inoperable. 46CFR182.410(b)

Fire Main System: Fire Pumps: The pump outlet must be fitted with a pressure gauge. Installed pressure gauge was found to be inoperable. 46CFR181.300

Vessel is required to undergo an annual inspection within 3 months before or after each anniversary date. Vessel's anniversary date is 09NOV and is overdue for annual inspection.

A vessel that is exposed to salt water more than three months in any 12 mo period must undergo a drydock exam and internal exam at least once every two years. Vessel last hull inspection is recorded as October 19, 2016. Vessel is overdue for credit drydock exam.

Make arrangments for CO2 fixed fire suppression system to release into both engines simultaneously.

Make arrangements to close all ventillation on both sides of engine compartment.

Make emergency hatch from aft storage area available for use by crew.

Make all emergency fuel shut-off valves fully operational.

Properly label all emergency fuel shut off valves.

Repair fractured weld on port side in fore-peak.

Properly label emergency escape hatch.

Replace grab line on life ring buoy.

Repair or replace fire pump pressure gauge IAW 46CFR181.300(b)

Make vessel available for an annual inspection to be conducted by a USCG Marine Inspector IAW 46CFR176.500(b)(1).

Provide approved metallic fuel piping for generators fuel lines in accordance 46 CFR 182.710 or an approved flexible hose that meets the SAE J 1942 standard in accordance with 46 CFR 182.720

Provide watertight seal for bulkhead penetrations below deck plated in engineroom aft watertight bulkhead.

Provide emergency broadcast placard with vessel specifics for bridge.

Remove/repair excess and burned electrical wiring from stbd engineroom bulkhead.

Replace missing watertight door gasket for lower aft watertight door.

remove excess abandoned wire from under bridge operating console.

Provide permanent wiring installations for bridge equipment with adequate circuit protection. Equipment is currently plugged into power outlet multipliers.

Provide proof of enrollment in Drug and Alcohol Testing Program

EPIRB Battery expired.

Ensure lifeboat painter is properly connected to the weak link.

Properly mount the 12 volt bus and 110 volt outlet under the dashboard.

Provide Copy of Stability Letter onboard vessel.

Provide current fire extinguisher servicing certificates for all portable and fixed firefighting systems.

Crop and renew areas on port bow below waterline where small doublers are located.

Complete annual servicing on firefighting equipment.
