PSCO received a report that the vessel parted one mooring line and three lines had completely unspooled from aft mooring whinches. Vessel is required to have class verify proper operation and structural integrity of aft mooring winches. 74 SOLAS (a) 2014 cons I/11(a)

The condition of the ship and its equipment shall be maintained to conform with the provision of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or person on board. PSCO received a report that the vessel broke free and collided with two barges and on towing vessel. PSCO observed damage to the port stern at the 11m line behind the gangway. Also observed damage around 1 meter above the water line around the house on the port side. Vessel should periodically sound tanks to ensure no ingress until confirmed by class the condition of the ship 74 SOLAS (a) 2014 cons. I/11(a)
