In tankers, electrical equipment, cables, and wiring shall not be installed in hazardous locations unless it conforms with standards not inferior to these acceptable to the organization. PSCO observed damaged and exposed electrical cable, in the hazardous zone, for the forward port cargo deck lighting on top of the fan room. Forward port light shall be electrically isolated and tagged out per SMS until properly repaired. 74SOLAS(20) II-2/45.11, 40 AC

The machinery and fittings shall be of a design and construction adequate for the service for which they are intended. While transiting in Lower Mississippi River, vessel experienced a reduction in propulsion and reported fuel leak on MDE #5 cylinder. Vessel was restricted to anchorage under COTP 440-20. It was determined that issue was caused by a damaged spring in the #5 fuel injection pump. Class surveyor attended vessel and attested to satisfactory repair and testing of MDE. 74 SOLAS (14) II-1/26.1
