The general construction of the vessel shall be such as to minimize fire hazards insofar as is reasonable and practicable. Two of the Class "A" doors leading into the fuel purifier room from the engine room, which is a Class "A" fire boundary, were found to be tied and held in the "open" position.

Resolution: Vessel removed hold-back ties on the doors, allowing for the proper function.

Whenever there is a hazardous condition either on board a vessel or caused by the vessel or its operation, the owner, agent, master, operator, or person in charge must immediately notify the nearest Coast Guard Sector Office. Containers were found on deck overflowing with old rollers from the vessel's conveyor belt system, as well as with scrap metal and other waste, with no means of containment to prevent the rollers and other scrap from becoming a safety hazard in rough seas or weather.

Closures for openings above the freeboard deck must be as strong as the structure to which they are attached and must be weathertight. A porthole on the starboard side leading into the accommodation space was found to have a hole cut in it with a hose for a portable AC unit duct taped to the outside.

The diamond, lines, and letters must be painted in white or yellow on a dark ground or in black on a light ground and permanently marked on the sides of the vessel. The load line was found to be deteriorated to the point where it no longer contrasted with the background.

When alteration or repair of boilers, pressure vessels, machinery, safety valves or piping systems becomes necessary, the work shall be done under the cognizance of the Officer in Charge of Marine Inspection. It shall be done in accordance with the regulations in effect at the time such vessel or installation was contracted for or built (whichever is latest), or in accordance with the regulations in effect for new construction. An automated valve controlling the temperature on the cooling water system for the main propulsion system, as well as other auxiliary machinery in the engine room, was found to be sheared off, and the crew had rigged a manual lever and chain fall system to control the valve. No notification was made to Lloyd's Register or the Transport Canada.

It shall be done in accordance with the regulations in effect at the time such vessel or installation was contracted for or built (whichever is latest), or in accordance with the regulations in effect for new construction. A hose was found rigged on top of the main engine scrubber to drain excess water produced by the scrubber into a bucket. No notification was made to Lloyd's Register or to Transport Canada

Vessel reported loss of power, which resulted in loss of propulsion of the main engines while transiting near Gros Cap Reef Light in Lake Superior, Michigan. Vessel identified the cause was their fuel pump on their service generator. Conduct appropriate repairs prior to weighing anchor from Waiska Bay to the satisfaction of U.S. Coast Guard. (46 CFR 58.01-35)

Resolved 20 May 2021
Resolution: Vessel had a spare fuel pump on board and changed out the faulty unit with the new one. Conducted satisfactory tests on all generators and engines.
