Each vessel vapor connection flange face must have a permanent stud projecting outward that has a 12.7 millimeter (0.5 inch) diameter and is at least 25.4 millimeters (1 inch) long. It must be located at the top of the flange face, midway between boltholes, and in line with the bolthole pattern.
PSCO observed no permanent stud located on any of the vapor connection flange faces at port and starboard manifolds of vessel.

PSCO found a 2 inch diameter hole in the "A" class bulkhead bewteen the main machinery space and steering gear space and another 2 inch diameter hole in the "A" class bulkhead between deck foam room and a storage space.

0800-Accident Prevention-Each cargo tank of a tankship must be equipped with intrinsically safe high level alarm and a tank overfill alarm. The tank overfill alarm must be independent of the cargo gauging system. Both high level alarm and overfill alarms are inoperable on cargo tanks; 2 port and starboard, and 8 port and starboard. Vessel not permitted to load in the tanks listed above until repaired or replaced. 46 CFR 39.2007 - 50 c.
