An immersion suit shall be constructed with waterproof materials such that: It can be unpacked and donned without assistance within 2 minutes, taking into account donning of any associated clothing, donning of a life jacket if the immersion suit is to be worn in conjunction with a life jacket to meet the requirements of paragraph 2.3.1.2, and inflation of orally inflatable chambers if fitted. PSCO observed egineers, officers and crew memebers failed donning immersion suits within 2 minutes. 74SOLAS(20) III/34 LSA code 2.3.1.1.1

Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. PSCO identified loose connection fittings for the lines connected to the bottle head assembly on all bottles in the row starting with bottle #609. PSCO further identifed multiple CO2 bottles throughout the CO2 room with the same loose connection fittings.

Resolution: Technician from Survitec attended the vessel for checking the system. All loose hoses
for all cylinders were tightened,checked and reported satisfactory by the service company.
MARPOL ANNEX I/14.7
Oil filtering equipment referred to in paragraph 2 of this regulation shall comply with paragraph 6 of this regulation. In considering the design of such equipment and approvals, the Administration shall have regard to the specification recommended by the organization. Oil content meter was continuously reading 06 to 07 ppm. After multiple attempts utilizing the fresh water calibration and cleaning, the ppm did not change to 00 to 02 ppm as indicated in the manual. OWS was set at 05 or less ppm to discharge effluent overboard. PSCO did not witness the three way valve engage during the operational test.
