46 CFR 119.200 (b) All engines must have at least two means for stopping the engine(s) under any operating conditions. The fuel oil shutoff required at the engine by § 119.455(b)(3) of this part will satisfy one means of stopping the engine. 46 CFR 119.455 (b)(3) Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. If the handle to the shutoff valve at the tank is located inside the tank compartment, it must be located so that the operator does not have to reach more than 300 millimeters (12 inches) into the compartment and the valve handle must be shielded from flames by the same material the hull is constructed of, or some noncombustible material. Electric solenoid valves must not be used, unless used in addition to the manual valve. Remote Fuel Shutoff Valve, cable & valve arrangement, did not demonstrate valve closure satisfactorily.

46 CFR 121.620 All microprocessor or computer based systems must meet the requirements of part 62 in subchapter F of this chapter. 46 CFR 61.40-1 (a) All automatically or remotely controlled or monitored vital systems addressed by part 62 of this subchapter must be subjected to tests and inspections to evaluate the operation and reliability of controls, alarms, safety features, and interlocks. Test procedures must be submitted to the Coast Guard for approval.(b) Persons designated by the owner of the vessel shall conduct all tests and the Design Verification and Periodic Safety tests shall be witnessed by the Coast Guard.(c) Design Verification and Periodic Safety test procedure documents approved by the Coast Guard must be retained aboard the vessel. Several items/steps in the PSTP's did not produced designed results.

46 CFR 115.814 At each initial and subsequent inspection for certification the owner or managing operator shall be prepared to test the steering systems of the vessel and make them available for inspection to the extent necessary to determine that they are in suitable condition and fit for the service intended. Servo-type power systems, such as orbital systems, must be tested and capable of SMOOTH OPERATION BY A SINGLE PERSON in the manual mode, with hydraulic pumps secured. Emergency steering instructions were not satisfactorily posted.

On vessel of more than 65 ft in lenght, th epublic address system must be fixed installation and be audible during normal operations throughtout the accomidation space and all other spaces normally manned by crew members. The public address system while in ALL MODE does not sound in all spaces as intended. 46 CFR 121.610 (b)

The fire control plan must show escape routes, areas of refuge, embarkation stations... The fire control plan did not show areas of refuge, plan needs to be updated. 46 CFR 116.530

Complete but simple instructions for the operation of a fixed gas fire extinguishing system must be located in a conspicuous place at or near each pull box and stop valve control and in the space where the extinguishing agent cylinders are stored. Also include a schematic diagram of the system and instructions detailing alternate methods of releasing the extinguishing agent... The fixed fire extinguishing system diagram and operating procedures need to be at the pull location.

The certificate of inspection (COI)issued by an Officer in Charge, Marine Inspection (OCMI), to a vessel required to be inspected under 46 U.S.C. 3301 specifies the minimum complement of officers and crew necessary for the safe operation of the vessel. The COI of subject vessel states "A COPY OF THE DESIGNATION LETTER SHALL BE MAINTAINED ON BOARD THIS VESSEL." There is no letter on-board. A copy of the letter was provided and is place on-board Item cleared.

The owner, charterer, master, or managing operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months, as tot he duties that the crew member is expected to perform in an emergency including, but not limited to, the emergency instructions listed on teh emergency instruction placard required by 122.510, when applicable, the duties listed int eh station bill required by 122.514. Crew unable to identify void/space in which a bilge alarm sounded. Additionally, crew was not knowledgeable in operation of vessel's bilge system. Vessel's crew must express competence in emergency situations in the presence of CG Marine Inspector prior to vessel's movement.

Each hatch exposed to the weather must be watertight, hatches leading to void#2 and lazarette #2 are leaking and causing the high level water alarm to go off.

MI observed potable water connection located in an area with unrestricted access.

Vsl bilge alarm located in the lazarette was inoperable.

Vessel Drug and Alcohol Program (DAPI) documentation was not onboard. No Employee Assistance Program (EAP) phone numbers were posted. Ensure all DAPI documentation is onboard, and post EAP number on pilothouse and in crew lounge.

GOVERNORS I crew must perform man overboard drill to the satisfaction of the USCG attending MI.
