The owner or managing operator shall submit drawings, sketches, or written specifications describing the details of any proposed alterations to the cognizant [name withheld] alterations must be approved by the OCMI before work is started. MIs identified a gusset added to each pontoon near the outboard mount. The port side pontoon in the most aft section had over 3inches of standing water in it, and a one inch cracked weld near the repair. Prior to starting repairs, submit a proposal to the satisfaction of the attending MI with details on how to identify source of the water intrusion and plans to repair the cracked weld. 46 CFR 176.700(b)

Resolution: O/O had the un-approved(gussets) modification removed, cracks welding, and pressure tested the aft sections of the pontoons where the cracks were noted. MIs observed the O/O pressure test both pontoons to 1.5PSI for 10min and sprayed soapy water.

A vessel not required to have a power driven fire pump by § 181.300 must have at least three 2.5 gallon buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. The words “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. Vessels buckets were corroded beyond use. Replace to the satisfaction of MI. 46 CFR 181.610
