The machinery, boilers, and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for service which they are intended and shall be so installed and protected as to reduce to a minimum and danger to persons on board, due to regard being paid to moving parts hot surfaces and other hazards. PSCO conducted damage survey following propulsion irregularity and issue of COPT 0213-21 dated 08APR2021. Temperature controller set point for the main diesel forward cooling that controls the 3-way valve was changed from 80 degrees to 70 degrees.

Nautical charts and nautical publications shall be adequate and up to date. PSCO observed ship's primary means of charting, ECDIS, had not been updated since 12Jul18 (apprx.4 months). Crew stated that they could update charts by way of DVD or USB drive. Crew recieved last DVD update on 12Jul18. Crew stated that USB port did not work. Prior to movement, ECDIS must be updated and fully operational.

Vessel failed to report that the MF/HF radios were inoperable. Ship lacked proper procedures that detailed when ship needed to notify Coast Guard via eNOA of inoperable equipment. Ship must provide corrective action plan from company in concurrence with Flag State Approval.
