An enclosed or partially enclosed space within a vessel must be adequately ventilated in a manner suitable for the purpose of the space. Port void vent duct is damaged; replace with new. 46 CFR 177.600(a)

Materials used in piping systems must meet the requirements of this subpart and be otherwise acceptable to the cognizant [name withheld] water valve in stbd lazz was inop and wasted; replace valve.46 CFR 182.700

Port voids contained hundreds of non type approved PFD's. Removed non type approved life jackets from vessel.46 CFR 180.72(a)

Each life jacket must be approved in accordance with either § 160.002, § 160.005, or § 160.055 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.155 or 160.176. 15 child life jackets are SOLAS approved but not USCG approved. Provide 15 USCG approved child life jackets prior to carriage of child passengers.

Each life jacket carried on a vessel on oceans, coastwise, or Great Lakes route, must have a life jacket light approved in accordance with § 161.012 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.112. Each life jacket light must be securely attached to the front shoulder area of the life jacket. 24 lifejacket lights inoperable. 46 CFR 180.75

An adult life jacket must be provided for each person carried on board a vessel. 01 adult life jacket taken out of service because the core foam material did not rebound under compression. 46CFR180.71(a)

Each life float must be fitted with a lifeline, pendants, two paddles, a painter, and a light. Lifefloat floating light inoperable. 46CFR180.175(d)

At each initial and subsequent inspection for certification, the owner or managing operator must be prepared to conduct tests and have the vessel ready for inspection of its fire protection equipment, including the following: Inspection of each portable fire extinguisher, semi-portable fire extinguisher, and fixed gas fire extinguishing system to check for excessive corrosion and general condition; 01 fire extinguisher located at starboard side main deck accomodation space is not fully charged. Service or replace extinguisher. Vessel had more 2A extinguishers on board than required on COI. 46CFR176.810

Deck rails must be designed and constructed to withstand a point load of 91 kilograms (200 pounds) applied at any point in any direction, and a uniform load of 74 kilograms per meter (50 pounds per foot) applied to the top rail in any direction. The point and uniform loads do not need to be applied simultaneously. Aft Starboard side bulwark inner door handle is missing. The door opens inward and is being retained by line. 46 CFR 177.900

Provide STCW Vessel Security Officer training and credential endorsement for the Master.

Provide proof of DAPI compliance of a chemical testing program for the crew and current enrollment list.

Employers must ensure that they and their crewmembers meet the requirements of the employer's chemical testing program to determine compliance with the provisions of this part. Provide proof of compliance with a chemical testing program for the crew.

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Provide EPIRB registration for the EPIRB provided for the vessel and mounted on the bridge wing.

If a vessel is operated on international voyages subject to SOLAS requirements, it must undergo a drydock examination once every 12 months. Vessel must undergo a credit drydock exam prior to obtaining a COI.
