The venting system shall be provided with devices to prevent the passage of flame into the cargo tanks. They shall be provided with self-closing and tightly sealing covers. #3 starboard tank and #5 starboard P/V valves are not tightly sealed at the flange above the vacuum side of the valve, allowing pressure to escape. Vessel is cargo restricted in #3 starboard and #5 starboard cargo tanks until deficiency is rectified. 74 SOLAS (14) Ch. II-2/4.5.3.3

Resolution: A Class Report and two photographs were received attesting to the rectification of the deficiencies identififed with sealing rings of the gas free vent covers for 3S and 5S P/Vs.

0299 - Other (crew) - Every crew member with assigned emergency duties shall be familiar with these duties before the voyage begins. PSCO observed vessel crew were unfamiliar with launching arrangements and procedures for lifeboat. Crew took 20 minutes to launch the rescue boat. Upon identification of the cradle clamp pins, the crew was able to successfully launch both lifeboats under 5 minutes. SOLAS 74(a) 2014 cons. III/19.2.1 Action Taken 10c
