During cargo transfer operations, inland oil barges must have appropriate equipment and supplies ready for immediate use to control and remove on-deck oil cargo spills of at least one barrel. (b) The equipment and supplies must include— (1) Sorbents; (2) Non-sparking hand scoops, shovels, and buckets; (3) Containers suitable for holding recovered waste; (4) Emulsifiers for deck cleaning; and (5) Protective clothing. Items were missing or wasted

The cargo discharge piping of all tank vessels shall be tested at least once each year for tightness, at the maximum working pressure. Manifold not marked

Lifebuoys must have the following attachments and fittings: (i) At least one lifebuoy on each side of the vessel fitted with a buoyant lifeline that is— (A) At least as long as twice the height where it is stowed above the waterline with the vessel in its lightest seagoing condition, or 30 meters (100 feet) in length, whichever is the greater; (B) Non-kinking; (C) Not less than 8 millimeters (5/16 inch) in diameter; (D) Of a breaking strength which is not less than 5 kiloNewtons (1,124 pounds-force); and (E) Is, if synthetic, a dark color or certified by the manufacturer to be resistant to deterioration from ultraviolet light. (ii) At least one-half the total number of lifebuoys on the vessel must each be fitted with a self-igniting light approved under approval series 161.010. The self-igniting light may not be attached to the lifebuoys required by this section to be fitted with lifelines. Life bouy installed was wasted and life line was wasted also.

You must submit a written application for an inspection for certification to the cognizant OCMI. To renew a Certificate of Inspection, you must submit an application at least 30 days before the expiration of the tank vessel's current Certificate of Inspection. When renewing a Certificate of Inspection, you must schedule an inspection for certification within the 3 months before the expiration date of the current Certificate of Inspection. Complete required COI renewal inspection prior to carrying cargo.

A vessel being pushed ahead, not being part of a composite unit, shall exhibit at the forward end, sidelights and a special flashing light. Vessel did not have on board sides or special flashing yellow light. No ops during darkness or low vis.

The owner, or PIC of a tank vessel must ensure that portable fire extinguishers are inspected and maintained IAW NFPA 10. Fire extinguisher on board was not serviced or maintained within the past year.

Each hose also must be marked with the following, (1) Maximum allowable working pressure; (2) Date of manufacture; and (3) Date of the latest test required by 33 CFR 156.170. (g) The hose burst pressure and the pressure used for the test required by 33 CFR 156.170 must not be marked on the hose. Vessel's transfer hose was marked with the incorrect pressures.

All lifesaving appliances on tank vessels must be in accordance with subchapter W of this chapter. Lifebuoy on the vessel was in disrepair and removed

Entries shall be made in the records for tank barges with respect to the following: (1) Inspections and tests of firefighting equipment. Once every year. Vessel fire extinguishers had no record of servicing this year.

A ship, except a ship of less than 26 feet in length, must have a placard of at least 5 by 8 inches, made of durable material fixed in a conspicuous place in each machinery space, or at the bilge and ballast pump control station,

Each hose must be marked with one of the following: (1) The name of each product for which the hose may be used; or (2) For oil products, the words “OIL SERVICE”. There was no labeling on the hose.

Each tank vessel must undergo internal structural examinations (ISE) in accordance with Table 31.10-21(a). Vessel restricted from operations until completion of Internal Structural Exam (ISE). 46 CFR 31.10-21

Each vessel that transfers oil in a port subject to the jurisdiction of the US must have onboard a valid vessel response plan. Vessel's response plan expired on 26Feb2019.
