The EPIRB's battery is replaced after it is used, or before the date required by FCC regulations in 47 CFR part 80, whichever comes sooner. EPIRB battery expired. No operation outside 03 NM from shore until replaced. 46 CFR 185.728(b)

The initial inspection may include an inspection of the... yards, masts, spars, rigging, sails... The scope of the annual inspection is the same as the inspection for certification but in less detail unless the cognizant marine inspector finds deficiencies. Three welds on Fore-stay chain plate above deck may be fractured. Dye pen test in the presence of USCG marine inspector or provide qualified third party test results. 46CFR 176.402(c)(1) & 46CFR 176.500(b)(1)(ii)

A vessel is inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Two shackles for the main sheet running rigging on the aft end of the boom are wasted. 46 CFR 176.800(a)

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to have the vessel ready for inspections of the hull structure and its appurtenances. STBD Engine mount area had exposed FRP fibers that needs sealing. 46 CFR 176.802(a)(1)

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. Four first id kit were expired. 46 CFR 184.710

Each vessel must be equipped with at least two operable portable battery lights. One of these lights must be located at the operating station and the other at the access to the propulsion machinery space. Missing 01 portable light at operating station. 46 CFR 183.430

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including the following: (3) Inspection of the condition of the superstructure, masts, and similar arrangements constructed on the hull, and on a sailing vessel all spars, standing rigging, running rigging, blocks, fittings, and sails. Vessels running rigging for the boom was found frayed in multiple areas. Replace boom rigging to the satisfaction of the Marine Inspector. 46 CFR 176.802(a)(3)

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years. The vessel has not completed a drydock examination within the last two years. Conduct satisfactory drydock examination prior to the carriage of passengers. 46 CFR 176.600(c)(1)

A suitable metal marine type strainer, meeting the requirements of the engine manufacturer, must be fitted in the fuel supply line in the engine compartment. Strainers must be leak free. Strainers must be the type of opening on top for cleaning screens….Fuel filters fitted with bowls of other than steel construction must be approved by the Commandant and be protected from mechanical damage. Approval of bowls of other than steel construction will specify if a flame shield is required. MI observed flame shield missing from fuel filters in engine room. 46 CFR 182.455(b)(6)

Oceans, coastwise, limited coastwise, and Great Lakes routes. A vessel on an oceans, coastwise, limited coastwise, or Great Lakes route must carry— (1) Six hand red flare distress signals approved in accordance with § 160.021 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.121; and (2) Six hand orange smoke distress signals approved in accordance with § 160.037 in subchapter Q of this chapter, or other standard specified by the Commandant. Vessel missing required amount of orange and red distress flares. Vessel representative must replenish missing flares to the standard amount to the satisfaction of the Marine Inspector. 46CFR180.68(a)

Each EAP education program must include at least the following elements: display and distribution of informational material; display and distribution of a community service hot-line telephone number for crewmember assistance, and display and distribution of the employer's policy regarding drug and alcohol use in the workplace. Vessel does not display the required information on the vessel. Vessel rep must post the required information for the EAP education program to the satisfaction to the Marine Inspector. 46CFR16.401(a)

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information: (1) Date of the drill and training; and (2) General description of the drill scenario and training topics. Vessel had no records of conducting required abandon ship and man overboard drills. Vessel must create an appropriate log to record all required drills, to the satisfaction of the Marine Inspector. 46CFR185.520(f)

Firefighting drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information: (1) Date of the drill and training; and (2) General description of the drill scenario and training topics. Vessel had no records of conducting required fire drills. Vessel must create an appropriate log to record all required fire drills, to the satisfaction of the Marine Inspector. 46CFR185.524(d)

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. Vessel’s med kit was found with multiple expired medications. Vessel representative must replace all expired medications to the satisfaction of the Marine Inspector. 46CFR184.710

Waste receptacles must be constructed of noncombustible materials with no openings in the sides or bottom. Vessel was found with plastic waste receptacles. Vessel representative must replace the plastic receptacles with noncombustible receptacles. 46CFR177.405(f)

Each vessel must be equipped with at least two operable portable battery lights. One of these lights must be located at the operating station and the other at the access to the propulsion machinery space. Vessel did not have the required two portable lights. Vessel representative must install two portable lights on the vessel to the satisfaction of the Marine Inspector. 46CFR183.430

Each life jacket, immersion suit, and ring life buoy must be marked in clearly legible block capital letters with the vessel's name. Vessel’s ring life buoy was not clearly marked with the vessel’s name. Vessel rep clearly marked the life buoy, corrected on the spot. 46CFR185.604(b)

The words “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. Vessel’s fire buckets were found without any markings. Vessel representative properly marked the fire buckets as required, corrected on the spot. 46CFR181.610

Fuel lines must be accessible, protected from mechanical injury, and effectively secured against excessive movement and vibration by the use of soft nonferrous metal straps which have no sharp edges and are insulated to protect against corrosion. Vessel was found with loose fuel lines connecting to the fuel tanks. Vessel representative properly secured the fuel lines to the fuel tanks, during the inspection, correcting on the spot. 46CFR182.455(b)(3)

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections …on a sailing vessel all spars, standing rigging, running rigging, blocks, fittings, and sails. During inspection, Marine Inspector found that all shackles were unmoused. Mouse all shackles to prevent inadvertent release from rig pumping. 46 CFR 176.802(a)(3)

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Vessel shall ensure that at all times when vessel is underway, a crew member is on board holding the appropriate FCC marine radio operator permit. Vessel shall provide documentation to the satisfaction of Marine Inspector. 46 CFR 184.502

Each vessel must - (1) Have permanent draft marks at each end of the vessel; or (2) Have permanent loading marks placed on each side of the vessel forward and aft to indicate the maximum allowable trim and amidships to indicate the maximum allowable draft. Vessel shall make draft mark visible on vessel in accordance with it's stability letter during the next dry dock and to the satisfaction of the attending Marine Inspector. 46 CFR 185.602(c)

Adult life jackets must be provided for each person on board. Vessel missing 1 adult life jacket.

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. Life ring was found degraded, with core exposed. - 46 CFR 810(c)

When operating a vessel on a body of water where the discharge of untreated sewage is prohibited by the Environmental Protection Agency under 40 CFR 140.3, the operator must secure each Type III device in a manner which prevents discharge of sewage. Acceptable methods of securing the device include - (3) using a non-releasable wire tie to hold each valve leading to an overboard discharge in the closed position. MI found both overboard dump valves not locked in the closed position. - 33 CFR 159.7(c)(3)

A device must be provided to automatically shut down power ventilation serving the protected space and engines that draw intake air from the protected space prior to release of the extinguishing agent into the space. Port side intakes were missing closures. - 46 CFR 181.410(b)(10)

A vessel with a pre-engineered fixed fire extinguishing system must have the following equipment at the operating station: a light to indicate discharge, an audible alarm, and a means to reset devices automatically shut down. Vessel Captain was unable to override engine start upon simulated release of agent. Corrected on spot. - 46 CFR 181.420(b)
