Each cargo tank with a volume exceeding 20 m3 should be fitted with at least two pressure relief valves of approximately equal capacity, suitably designed and constructed for the prescribed service. Vessel reported a failure of one of their pressure relief valves for the #4 cargo tank.
A Class report attesting the the proper repair of the pressure relief valve and a causal analysis for the failure are required to be submitted prior to loading cargo into the #4 cargo tank.

The master gas fuel valve required by 16.9.7 should close automatically if the required air flow is not established and maintained by the exhaust ventilation system.
Unable to demonstrate MGV automatic closure with loss of ventilation to GCU gas room (gas hood). PSC examiners witnessed shut down of GCU gas room ventilation fans (02 fans) but witnessed MGV remain open. Indicator in CCR showed attempt to close (visual red indicator) but valve on deck did not indicate physical closure. Class to verify proper operation of all MGV automatic shut-downs required by IGC Code.
Action required: 17 - Rectify deficiencies prior to departure
