07123 - The following fire protection systems shall be kept in good order so as to ensure their required performance is a fire occurs. PSC observed the vessel's installed smoke extraction sample system had sample hose #9 disconnected. The disconnection prevented smoke detection from point #9 (holo #3 exhaust duct). The hole left from this diconnection also reduced extraction from all sample points. This condition poses a substantial deterioration of the systems ability to detect fires from all sample points. 74 SOLAS (2014), CH II-2, Reg. 14.2.1.2 - 17ac

01117 - Oil filtering equipment shall be of a design approved by the administration. PSCO observed that the vessel's IOPP states the capacity of the vessel's OWS is 10 cubic meters per hour. A review of the vessel's ORB and content meter showed this capacity was exceeded on operations on 09 May 2019 and 28 Nov 2018.
