After loading and prior to departure and at all other times necessary to assure the safety of the vessel, the master shall determine that the vessel complies with all applicable stability requirements in the vessel's trim and stability book, stability letter, Certificate of Inspection, and Load Line Certificate, as the case may be. The vessel may not depart until it is in compliance with these requirements. The vessel's maximum permitted draft at the bow was 5' 11-3/8" the observed draft at the bow was approximately 6'9". Vessel was not modified to prevent filling of ballast tanks. Vessel did not have a working tank plan aboard. 46 CFR 185.315 (old)

Vessels operated beyond the 20 nautical mile limitation specified in paragraph (a)(1) of this section, but not more than 100 nautical miles from the nearest land, must be equipped with a MF-DSC frequency transmitter meeting the requirements of § 80.1101(c)(3) and capable of transmitting J3E emission and a receiver capable of reception of J3E emission within the band 1710 to 2850 kHz, in addition to the VHF-DSC radiotelephone installation required by paragraph (a)(1) of this section. Vessel did not have a single side band radio. 47 CFR 80.905(a)(2)

Wiring methods and materials (50 volts or more). (a) All wiring and cables shall be suitable for marine service and for the particular installation used. Vessel had an abundance of dead end wires coiled up throughout the engine room. 46 CFR 183.10-20 (old)

Additional control equipment and measuring instruments shall be provided as necessary to ensure satisfactory operation of the generator or generators. Mechanical interlock would not stop the generator switches due to interlock being loose and switch being broken. 46 CFR 183.10-5 (old)

A vessel is inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Vessel had a seawater pump in engine room with cracked housing and was extremely corroded 46 CFR 176.800

Each unit must have hand covers, guards, or rails installed on all belts, gears, shafts, pulleys, sprockets, spindles, flywheels or other reciprocating, rotating or moving parts of machinery or equipment normally exposed to contact by personnel. Vessel had exposed steering equipment from missing guard. 46 CFR 108.223

On a vessel of more than 19.8 meters (65 feet) in length, the public address system must be a fixed installation and be audible during normal operating conditions throughout the accommodation spaces and all other spaces normally manned by crew members. Internal public address system inoperable. 46 CFR 184.610(b)

A 40-B portable fire extinguisher installed outside the space may be substituted for a fixed gas fire extinguishing system in a storeroom containing flammable liquids (including liquors of 80 proof or higher where liquor is packaged in individual containers of 9.5 liters (2.5 gallons) capacity or greater) or a paint locker, with a volume of not more that 5.7 cubic meters (200 cubic feet) Vessel had a 10-B fire extinguisher. 46 CFR 181.400(b)(3)

Each ring buoy with a floating waterlight must have a lanyard of at least 3' but not more than 6' securing the waterlight around the body of the ring life buoy. The waterlight was secured to the grab line. 46 CFR 180.70(d)(2)

Cylinder requirements. Cylinders used for containing hazardous ships' stores that are compressed gases must be maintained and retested in accordance with 49 CFR 180. — Vessel had an oxygen tank that showed a last hydro date of 10/12. 46 CFR 147.60

Covered metal trash containers shall be provided in lounge areas and the spaces shall be maintained to minimize fire and safety hazards and to preserve sanitary conditions. Vessel had combustible plastic trashcans on board 46 CFR 177.30-7(d) (Old)

Have a nozzle that has a 1/2 inch solid stream orifice or a combination nozzle approved by the Commandant in accordance with 162.027-6 of this Chapter. The nozzle must be bronze or another material that has strength and corrosion resistance properties equal to those of bronze. Vessel had a plastic nozzle. 46 CFR 181.15-10(d)(2) (Old)

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date charts of large enough scale to make safe navigation possible. Vessel did not have any paper charts, Rose Point ECS does not meet equivalency as per NVIC 01-16 CH-3 (RTCM Class B ECS). 46 CFR 184.420 (a) (1)

Class 1 doors are permitted within passenger, crew, and working spaces wherever located. if- The vessel is less than 150 gross tons and will not proceed more than 20 nautical miles from shore. Vessel has a watertight door on a watertight engine room bulkhead and route beyond 20NM, vessel's stability letter indicates there are no watertight doors. Owner could not provide evidence the WTBH was not required to comply with flooding or damage stability regulations. COI/Stability letter allows cargo in excess of that allowed by the crane installation letter of 19 April 2022. 46 CFR 170.24 (b) (2) (Old)

MI's found ballast water in Ballast 2 P&S, and Ballast 3 P&S. FW 1 P&S could not be confirmed. 46 CFR 185.315 (old)

Where flexible nonmetallic hose is permitted for use in piping systems by this section, it must meet SAE J-1942 (incorporated by reference; see 46 CFR 175.600) or be specifically approved by the Commandant. The following restrictions apply: Vessel MDEs fuel hoses were Aeroquip GH 781-10 and did not have the required 624-18 Fire Sleeve for fuel service. 46 CFR 182.720(e)

Fire extinguishing equipment installed on a vessel in excess of the requirements of §§ 181.400 and 181.500 must be designed, constructed, installed, and maintained in accordance with a recognized industry standard acceptable to the Commandant (CG-ENG-4). Vessel had Ellide Fire Fire Extinguishing Balls onboard which were not USCG Approved. 46 CFR 181.120(a)

All vessels shall be provided with bilge pumps in accordance with Table 182.25-10(a). (Over 65 feet needs 2 fixed power pumps). Vessel was missing a bilge pump. Install bilge pump to the satisfaction of the marine inspector. 46 CFR 182.25-10(a) "OLD T"

A bilge pipe piercing the collision bulkhead must be fitted with a screw-down valve located on the forward side of the collision bulkhead and operable from the weather deck. Vessel screw-down valve was broken. Repair screw-down valve to make it operable from the weather deck. 46CFR182.510(d)

vessel ready for inspections of machinery, fuel, and piping systems, including all machinery essential to the routine operation. Port and Starboard shaft seals were leaking water and need to be replaced. Replace shaft seals prior to next credit dry dock. 46CFR176.804(c)

Fuel fill pipes shall terminate on the weather deck clear of coaminngs and shall be fitted with shutoff valves, watertight deck plates or screw caps suitably marked for identification. Vessel was found with cam lock fittings.46CFR182.20-30(c)

Hydrostatic tests under pressure . Each pressure vessel ... must be subjected to a hydrostatic test at a pressure of 1.25 times the maximum allowable working pressure twice within any five-year period. Pressure vessel has exceeded its required hydrostatic test due date. 46CFR61.10-5(d)

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years. Vessel missed it's drydock that was due 31MAR23. Vessel must complete drydock examination prior to carrying passengers. 46 CFR 176.600

Stowed so that when the vessel sinks the survival craft floats free and, if inflatable, inflates automatically. Both life rafts were found improperly attached to weak link. 46 CFR 180.130(a)(2)

All electrical installations shall be to the satisfaction of the [name withheld] main engine has exposed wiring on the RPM sensor. Repair/replace to the satisfaction of attending MI.46 CFR 183.01-15(a)"OLD-T"

The setting of safety valves or relief valves on boilers and unfired pressure vessels shall be checked. Pressure relief valves for both unfired pressure vessels would not re-seat after manually lifted. Repair/replace to the satisfaction of attending MI. 46 CFR 176.25-39(b)(2)"OLD-T"

Each hatch exposed to the weather must be watertight. Engine room escape hatch does not seal properly making it non-water/weathertight. Repair/replace to the satisfaction of the attending MI. 46 CFR 171.124(a) "OLD-T"

Watertight door that gives access into the hull shall be watertight. Knife edge of WTD from main deck to tool room has wastage on bottom edge. Repair/replace to the satisfaction of the attending MI.46 CFR 171.124(e) "OLD-T"

Penetrations in watertight bulkheads must have means to make them watertight. Electrical penetration forward bulkhead of tool room found not watertight. 46 CFR 171.114(a)(2) "OLD-T"

At each inspection for certification all observed unsafe practices must be corrected and in a satisfactory condition. Both shaft seals have excessive leaking. Repair/replace to the satisfaction of attending MI. 46 CFR 176.830(a)

Forward masthead light was inoperable. Repair masthead light and prove proper operation.

Vsl does not have proper distress flares and smoke signals, or substitutes. Acquire 06 hand red flare distress signals and 06 hand orange smoke distress signals. Further substitutions are listed in 46 CFR 180.68(c).

Vsl needs life jacket placards representing every type of life jacket that is onboard the vessel. Provide proof that the various types of life jackets onboard have a respective placard that is visible to the crew and passengers.

The outer insulation of main passageway emergency light cable was damaged and the wiring was exposed. Replace cable.

Remove extension cord pwr supply for freezer on aft deck; hardwire freezer to pwr supply w/appropriate circuit protection. Provide plan for review prior to installation. Due April 11, 2021. 46 CFR 176.700, 46 CFR "Old T" 183.10-20(a)(2)

Cap off empty pipe in ER port side at top of blkhd formerly used as wire/pipe blkhd penetration and small dia (~1") penetration in same blkhd towards top-center of blkhd. Due April 11, 2021. 46 CFR "Old S" 171.040

Vessel did not have fire/abandon ship drill records onboard.

Vessel is required to have the generators circuit breakers to be arranged so as to prevent their simultaneous connection to the switchboard. Marine Inspectors discovered that one of the breaker switches had broken off rendering the interlock device ineffective. Replace the breaker switch so the interlock device can function as designed.

Vessel is required to have electrical equipment installed suitable for the roll and pitch of the vessel underway. Marine Inspectors discovered the washer and dryer installation on board was not suitably secured. Securely mount the washer and dryer in place to remove hazard to crew.

Vsl is broadcasting: Invalid Call Sign.

Prove proper operation of bilge pickup in lazarette.

Renew FCC ship station license, FCC radio safety certificate, FCC Bridge to bridge radio certificate.

Replace outlet in engine room that charges emergency light.

Place pressure guage on fire pump outlet.

Provide all required flares.

RT01: Provide thickness readings of the 04 Locations as discussed with vessel rep (Collision bulkhead, 02 ER Locations, and port ER Hull). If repairs are being conduct, submit plans.

RT02: Vacuum ER bilge to be ready for inspection.

RT03: Clean out valve uptakes to be free of debris.

RT04: Replace shaft bearings and rudder bearings as discussed.

Hydrostatically test 2 air receivers to 1.25x MAWP in accordance with 46CFR61.10-5(a). Due PTCDD.

Provide bundling and metal supports for wiring on the top deck (above wheelhouse). Due PTCDD.

Vessel must have emergency lighting illuminating the emergency exits. One emergency exit light missing in the galley and one in the emergency escape hatch from the engine room.

EPIRB Hydrostatic release unit has expired. Replace with new one.

Alcohol test swabs have expired. New batch has been ordered but not received at time of inspection.

CC01: Remove Water from Forepeak, make ready for inspection.

CC02: Remove water, all dirt, and trash from FWD void bilge. Make ready for inspection.

CC03: FWD STBD Ballast, Remove 02 Plates tack welded to the hull.

CC04: Remove water and clean bilge in AFT void under water tank and shaft tubes. Make ready for inspection.

CC09: Crop collision bulkhead from the keel up to good steel as marked in space.

CC10: Crop the center line beam from the stem to the collision bulkhead, from the keep up to the top. Submit repair proposal.

CC08: Crop Port & STBD Side hull plate in the forepeak from the stem leading aft at least 6" past the collision bulkhead into the FWD void from keel up to existing weld. Submit Repair Proposal.

CC11: Provide 3rd party to conduct ultra sonic testing of hull plating in a 2ft grid between bulkheads 8 & 9 and 1 & 2 from port chine to STBD chine.
