Vessel had a temporary service generator on the back deck which was tied into the shore based connection with the capability of tying into the vessels main switch board. Installation was not approved by administration.

Every company should develop, implement and maintain a safety management system which includes the following compliance with relevant international and flag state legislation. Defined lines of communication between shore and shipboard personnel and procedures for reporting non-conformities. The vessels SOLAS and load line certificates and regulatory required vessel response plan and COFR became invalid or expired while at sea conducting local operations in US waters. The vessel remained at sea for four days continuing operations and then arrived in port with non-compliance with international and flag state legislation. The Master failed to report a non-conformity to the company. The company failed to communicate to the vessel it was in non-compliance with the international and flag state legislation.

Fire Fighting › Ventilation Systems › Ducts

The main inlets and outlets of all ventilation systems shall be capable of being closed from outside the space in the event of a fire. A starboard rear ventilation duct on the forecastle deck for a space inside the ship has electrical wires running through the system. This makes it so the ventilation duct may not be closed in the case of a fire.
